摘　　要：[目的]了解不同地区基本公共卫生服务项目社区诊断报告现状以及社区诊断报告实施现况。[方法]分层整群随机抽样共选取69个基层医疗卫生机构(卫生院、社区卫生服务中心)的社区诊断报告,描述性分析报告相关指标。[结果]社区诊断报告撰写主体多为当地疾控中心(53.63%)和县(区)级卫生健康局(33.33%);数据来源多为年鉴数据,监测资料来源以死因监测(82.6%)和慢性病监测(18.26%)占比较高,医院信息系统和居民个人健康档案系统数据(20.29%、34.78%)的应用占比偏低,无医保系统来源数据;样本机构的社区诊断报告对于人口社会学、辖区基本情况覆盖较为全面,对于期望寿命(47.83%)、疾病谱分析(57.97%)、传染病的发病情况(52.17%)以及健康素养(11.59%)、睡眠(15.94%)、心理健康(8.69%)等生活行为指标的报告占比较低,针对学生健康状况,报告占比偏低。[结论]各地在基本公共卫生服务项目社区诊断报告上进行了积极探索,但在撰写机构、数据来源、报告内容等方面还存在较大差异,亟待出台行业或地方标准,以提升社区诊断报告规范性和应用成效。Objective Understand current status of community diagnosis reports(CDR)of basic public health service projects in different regions and the implementation status of community diagnosis reports.Methods A total of 69 community diagnosis reports from primary medical and health institutions(rural township health centers and urban community health service centers)were selected by stratified cluster random sampling,and relevant indicators were reported by descriptive analysis.Results The main responsible bodies of CDR were mostly local CDC(53.63%)and county-level health bureau(33.33%);Data sources were mostly from yearbook data,monitoring data sources had high proportion in death causes monitoring(82.6%)and chronic disease monitoring(18.26%).The proportion of application in hospital information systems(20.29%)and residents personal health record systems(34.78%)were relatively low.There was no data from the medical insurance system.The CDR of the sample institutions covered the population sociology and the basic situation of administrative area comprehensively.The proportion of the report on life behavioral indicator such as life expectancy(47.83%),disease spectrum analysis(57.97%),morbidity of infectious diseases(52.17%),health literacy(11.59%),sleep(15.94%),mental health(8.69%)and other was relative low,and as well in the report for health status of students.Conclusions CDR for basic public health service projects in various regions has been actively explored,but there are large differences in various aspects such as writing institutions,data sources and report content.It is urgent to issue industry or local standards to improve the standardization and application effectiveness of CDR.
