摘  要:目的探究高原地区动态监测健康新生儿出生1~30 min血氧饱和度(SpO_(2))变化意义及对用氧指导价值。方法选取2019年1月至2021年6月高原地区健康新生儿212例,均采用脉氧饱和度监测仪测定出生1~30 min SpO_(2)水平变化。结果212例健康新生儿出生1 min、2 min、3 min、4 min、5 min、10 min、30 min任意下肢、右上肢SpO_(2)水平呈逐渐升高趋势;随着海拔升高,健康新生儿出生后相同时间点任意下肢、右上肢SpO_(2)水平呈下降趋势(P<0.05);随着出生时间延长,相同海拔地区健康新生儿任意下肢、右上肢SpO_(2)水平呈升高趋势(P<0.05);海拔·时间点交互作用差异有统计学意义(P<0.05);健康新生儿出生1 min、2 min、3 min、4 min、5 min、10 min任意下肢、右上肢SpO_(2)水平与海拔呈负相关(P<0.05);相同海拔地区不同性别、分娩方式健康新生儿出生1 min、2 min、3 min、4 min、5 min、10 min、30 min任意下肢、右上肢SpO_(2)水平无显著差异(P>0.05);海拔2000 m以下的健康新生儿出生1 min、2 min、3 min、4 min、5 min、10 min任意下肢、右上肢SpO_(2)水平与指南推荐导管前目标SpO_(2)水平的数值范围基本相符;海拔2000 m以上的健康新生儿出生1 min、2 min、3 min、4 min、5 min、10 min任意下肢、右上肢SpO_(2)水平低于指南推荐导管前目标SpO_(2)水平。结论随着高原地区海拔不断升高,新生出生1~10 min SpO_(2)水平逐渐低于指南推荐水平,30 min时SpO_(2)可达到正常水平,故在高原地区应结合新生儿临床表现给予窒息复苏,需谨慎用氧,以防出现低氧或高氧损害。Objective To explore the significance of dynamic monitoring of blood oxygen saturation(SpO_(2))changes in healthy newborns from 1 to 30 minutes after birth in plateau areas and their value for oxygen guidance.Methods A total of 212 healthy newborns in plateau areas from January 2019 to June 2021 were selected.Pulse oxygen saturation monitors were used to measure the changes in SpO_(2) levels from 1 to 30 minutes after birth.Results 212 healthy newborns were born at 1 min,2 min,3 min,4 min,5 min,10 min,and 30 min.SpO_(2) levels of any lower limbs and right upper limbs showed a gradual increase.As the altitude increases,the SpO_(2) levels of any lower limbs and right upper limbs of healthy newborns at the same time point after birth show a downward trend(P<0.05);as the birth time increases,the SpO_(2) levels of any lower limbs and right upper limbs of healthy newborns in the same altitude area show a downward trend.Increasing trend(P<0.05);the difference in the interaction between altitude and time point is statistically significant(P<0.05);healthy newborns are born 1 min,2 min,3 min,4 min,5 min,10 min,any lower limbs.The SpO_(2) level of the right upper limb was negatively correlated with altitude(P<0.05);healthy newborns with different genders and delivery methods in the same altitude area were born 1 min,2 min,3 min,4 min,5 min,10 min,30 min,any lower limbs.There was no significant difference in the SpO_(2) level of the right upper limb(P>0.05).The SpO_(2) level of any lower limb and right upper limb of healthy newborns under 2000 m above sea level was born at 1 min,2 min,3 min,4 min,5 min,10 min,and the SpO_(2) level of the guideline recommended before the catheter;healthy newborns above 2000 m above sea level The SpO_(2) level of any lower extremity and right upper extremity was lower than the guideline recommended target SpO_(2) level before the catheter at 1 min,2 min,3 min,4 min,5 min,10 min.Conclusion With the increasing altitude in high altitude areas,the SpO_(2) level of newborns at 1-10 min is gradually
