摘 要:目的维生素K1、K2与婴儿出血性疾病密切相关,而维生素K1、K2在新生儿中的浓度尚缺乏大样本调查。同时对新生儿维生素K1、K2水平的预测研究较少。亟需风险预测模型预判新生儿维生素K1、K2水平,从而为治疗掌握主动权。方法本文通过回顾性调查双中心共1587名患儿,应用相关性分析、单因素分析、Logistic回归分析得出风险因素,应用R4.0.3软件进行Logistic回归可视化,构建诺谟图,得出预测模型。结果相关性分析(维生素K1vs维生素K2)提示Pearson相关系数R:0.3376(95%CI:0.2932,0.3805),P=0.0001;Logistic回归分析:在维生素K1中,有孕妇妊娠并发症的孕妇所产的新生儿,有较高维生素K1水平,OR(95%CI)=1.838(1.060,3.158),P=0.03。在维生素K2中,孕周<37周,体重<2500 g,有孕妇妊娠并发症的孕妇所产的新生儿,有较高维生素K2水平,OR(95%CI)分别为:0.367(0.225,0.599)、0.569(0.377,0.859)、1.630(1.052,2.526),P均<0.05。结论新生儿维生素K1、K2总体处于低水平状态。维生素K1分布水平差异较大,维生素K2分布水平较为集中。维生素K1与K2具有很好的相关性。低体重、早产儿有更高的维生素K2。孕期并发症与维生素K1、K2升高有关。Objective Vitamin K1 and K2 are closely related to infant hemorrhagic diseases,while the concentration of vitamin K1 and K2 in newborns is still lacking in large samples.At the same time,there are few studies on vitamin K1,K2and clinical related factors.Methods This article retrospectively investigated 1587children and used correlation analysis,single factor analysis,Logistic regression analysis and statistics to draw conclusions.Nomogram was used to visualized the outcome of Logistic analysis.Results Correlation analysis(1589 for vitamin K1 vs 1589 for vitamin K2)suggested that Pearson’s correlation coefficient R:0.3376(95%CI:0.2932,0.3805),P=0.0001;Logistic regression analysis:among vitamin K1,newborns born to pregnant women with pregnancy complications have higher vitamin K1 levels,OR(95%CI)=1.838(1.060,3.158),P=0.03.Among vitamin K2,the gestational age is less than 37 weeks,the weight is less than 2500 g,and the newborns born to pregnant women with pregnancy complications have higher levels of vitamin K2.The OR(95%CI)is 0.367(0.225,0.599),0.569(0.377,0.859),1.630(1.052,2.526),P<0.05.Conclusion Newborns’vitamin K1 and K2 are generally at a low level.The distribution level of vitamin K1 is quite different,while the distribution level of vitamin K2 is more concentrated.Vitamin K1 and K2 have a good correlation.Low-weight,premature babies have higher vitamin K2.Complications during pregnancy are related to elevated vitamin K1 and K2.
