摘  要:目的探讨基于分段读出平面回波成像的扩散张量成像(rs EPI-DTI)泪腺定量测量在甲状腺相关眼病(TAO)活动性评估中的价值。资料与方法回顾性分析2017年2—11月于南京医科大学第一附属医院接受rs EPI-DTI眼眶检查的17例(34眼)活动期及13例(26眼)非活动期TAO患者,比较两组表观扩散系数(ADC)及部分各向异性分数(FA)图像上泪腺ADC值和FA的差异,采用受试者工作特征曲线分析各参数区分TAO患者是否处于活动期的效能。结果活动期TAO患者的泪腺FA明显小于非活动期患者(0.27±0.05比0.33±0.05;t=-4.873,P<0.001),ADC大于非活动期患者[(1.59±0.33)×10^(-3)mm^(2)/s比(1.42±0.18)×10^(-3)mm^(2)/s;t=2.563,P=0.013]。泪腺FA、ADC及两者联合诊断TAO患者分期的曲线下面积分别为0.824、0.622、0.861,敏感度分别为88.2%、44.1%、91.2%,特异度分别为73.1%、92.3%、65.4%。结论基于rsEPI-DTI图像的泪腺ADC与FA均可用于评估TAO的临床活动性,两者联合可以提高评估效能。Purpose To assess the value of lacrimal glands in evaluating disease activity of thyroid-associated ophthalmopathy(TAO)with readout-segmented echo-planar imaging based diffusion tensor imaging(rsEPI-DTI).Materials and Methods From February to November 2017,17 active(34 eyes)and 13 inactive(26 eyes)TAO patients who underwent rsEPI-DTI in the First Affiliated Hospital of Nanjing Medical University were retrospectively enrolled.Fractional anisotropy(FA)and apparent diffusion coefficient(ADC)values of lacrimal glands were measured and further compared between active TAO group and inactive TAO group.Receiver operator characteristic curve analysis was used to determine the efficiency of differentiating ability of these parameters.Results FA values of lacrimal glands in active TAOs were significantly lower than those of inactive TAO(0.27±0.05 vs.0.33±0.05;t=-4.873,P<0.001),while ADC values of active TAO were significantly higher than those of inactive TAO[(1.59±0.33)×10^(-3)mm2/s vs.(1.42±0.18)×10^(-3)mm2/s;t=2.563,P=0.013].The area under curve values of FA,ADC and combination of the two parameters were 0.824,0.622 and 0.861,respectively;the corresponding sensitivity was 88.2%,44.1%and 91.2%,respectively,the specificity was 73.1%,92.3%and 65.4%,respectively.Conclusion The ADC value and FA value of lacrimal glands based on the rsEPI-DTI could evaluate the disease activity of TAO,improving evaluation performance with combing FA and ADC.
