高频超声评价无孕产史育龄女性腹直肌间距的影响因素

摘　　要：目的应用高频超声评价影响无孕产史女性腹直肌间距的相关因素。资料与方法前瞻性连续性收集2018年3月—2020年3月于河南省人民医院超声科体检的正常无孕产史育龄女性624例。将腹直肌间距与年龄、身高、体重、体重指数(BMI)、腹直肌厚度进行线性回归分析。按照年龄将受检者分为3组:第1组(20~29岁)、第2组(30~39岁)、第3组(40~49岁);按照BMI分为3组:BMI 1组(BMI<18.5 kg/m^(2))、BMI 2组(BMI 18.5~24.9 kg/m^(2))、BMI 3组(BMI 25.0~29.9 kg/m^(2))。比较不同组间腹直肌间距的差异,分析影响腹直肌间距的因素。结果年龄和BMI是腹直肌间距的独立危险因素(回归系数分别为0.23、0.10,95%CI 0.11~0.34、0.25~0.56,t=3.94、5.07,P<0.01)。腹直肌厚度是腹直肌间距的独立保护因素(回归系数为-0.88,95%CI-1.30~0.46,t=-4.13,P<0.01)。体重可能对腹直肌间距产生一定影响(回归系数为0.13,95%CI 0.07~0.19,t=4.35,P<0.01)。身高与腹直肌间距无相关性(t=-0.07,P=0.95)。按照年龄分组两两比较第3组腹直肌间距明显大于第1组(P<0.01)和第2组(P<0.01);BMI 3组腹直肌间距明显大于BMI 1组(P<0.01)。结论超声可以客观评价无孕产史育龄女性腹直肌间距。年龄、体重、BMI、腹直肌厚度均是影响腹直肌间距的因素,而身高与腹直肌间距无明显相关。Purpose To evaluate the related factors affecting the inter-rectus distance in women without a history of pregnancy by highfrequency ultrasound.Materials and Methods 624 women of childbearing age without a history of pregnancy were continuously collected to perform normal sonography between March 2018 and March 2020 in Department of Ultrasound,Henan Provincial People’s Hospital.The linear regression analysis was conducted between inter-rectus distance and age,height,weight,body mass index(BMI),rectus abdominis thickness.Subjects were divided into three groups according to age:the first group(20-29 years old);the second group(30-39 years old);the third group(40-49 years old).All subjects were also divided into three groups according to BMI:BMI 1 group(BMI<18.5 kg/m^(2));BMI 2group(BMI 18.5-24.9 kg/m^(2));BMI 3 group(BMI 25.0-29.9 kg/m^(2)).The differences of inter-rectus distance among different groups were compared,the factors affecting rectus abdominis spacing were analyzed.Results Age and BMI were independent risk factors of inter-rectus distance(the regression constants were 0.23 and 0.10,95%CI were 0.11-0.34 and 0.25-0.56,t=3.94 and 5.07,P<0.01).The rectus abdominis thickness was independent protective factors for inter-rectus distance(the regression constant was-0.88,95%CI was-1.30--0.46,t=-4.13,P<0.01).The weight might play certain role in inter-rectus distance(the regression constant was 0.13,95%CI was 0.07-0.19,t=4.35,P<0.01).The height had no significant correlation with the inter-rectus distance(t=-0.07,P=0.95).The inter-rectus distance in the third groups,grouped by age,was significantly larger than those of the first and second groups(P<0.01).The inter-rectus distance in the BMI 3 group was significantly larger than those of the BMI 2 group(P<0.01)and BMI 1 groups(P<0.01).Conclusion The ultrasound can externally evaluate the inter-rectus distance in women of childbearing age without a history of pregnancy.Age,weight,BMI and rectus abdominis thickness are all factors that influence inter-rectus dista
