生肌愈疡膏联合负压封闭引流治疗蛇伤溃疡20例

摘　　要：目的:观察生肌愈疡膏联合负压封闭引流(VSD)治疗蛇伤溃疡疗效。方法:将已经确诊的蛇伤溃疡的住院患者60例按随机数字表法分为观察组、对照组1、对照组2,各20例。观察组采用外敷生肌愈疡膏联合负压封闭引流,两组对照组分别为单独使用生肌愈疡膏换药以及单独使用VSD负压引流。观察3组患者治疗前后中医证候积分变化、疮面恢复情况、创面碱性成纤维细胞生长因子(bFGF)和表皮细胞生长因子(EGF)水平的变化临床疗效。结果:治疗后,3组中医证候积分均下降(P<0.05),观察组优于两组对照组(P<0.05);观察组的肉芽覆盖时间较两组对照组缩短、住院次数以及换药次数比两组对照组减少(P<0.05);3组创面bFGF和EGF水平明显增高(P<0.01),并且观察组指标增长水平明显优于两组对照组(P<0.01)。总有效率观察组优于两组对照组(P<0.01)。结论:生肌愈疡膏联合负压封闭引流(VSD)治疗蛇伤溃疡能够促进创面生长、缩短病程并减轻患者的痛苦。Objective:To observe the curative effect of Shengji Yuyang Ointment combined with Vacuum Sealing Drainage(VSD)in the treatment of snake wound ulcer.Methods:Sixty cases of patients with snake wound ulcer were divided into observation group,control group 1 and group 2 with 20 cases in each group according to random number table method.The observation group was treated with external application of Shengji Yuyang Ointment combined with negative pressure drainage,and the control group 1 was treated with dressing change of Shengji Yuyang Ointment alone,the control group 2 with VSD alone.The changes of TCM syndrome scores,wound recovery,basic fibroblast growth factor(bFGF)and epidermal cell growth factor(EGF)levels in 3 groups were observed before and after treatment.Results:After treatment,the TCM syndrome scores of the three groups decreased(P<0.05),the observation group was better than the two control groups(P<0.05);the granulation coverage time in the observation group was shorter than that in the control group,and the hospitalization times and dressing change times in the observation group were lower than that in the control group(P<0.05);the levels of bFGF and EGF in the three groups were significantly increased(P<0.01),and the index growth level of the observation group was significantly better than that of the two groups(P<0.01).The total effective rate of observation group was better than that of control group(P<0.01).Conclusion:Shengji Yuyang Ointment combined with VSD in the treatment of snake wound ulcer can promote the growth of wound surface,shorten the course of disease and alleviate the pain of patients.
