耳尖放血联合耳穴贴压对后循环缺血性眩晕的疗效观察

摘　　要：目的:观察耳尖放血联合耳穴贴压对后循环缺血性眩晕的疗效。方法:选取临床确诊且符合纳入标准的后循环缺血性眩晕患者120例,随机分为观察组和对照组,各60例。对照组予以内科基础对症治疗,观察组在对照组基础上加耳尖放血联合耳穴贴压治疗,2周为1个疗程,分别评价治疗后两组患者眩晕改善情况,对比两组患者治疗前后椎动脉、基底动脉TCD的血流动力学变化及血液流变学变化,并追踪随访治疗后3个月、6个月的复发率。结果:两组患者治疗后眩晕情况均有所改善,且观察组患者的痊愈率及有效率均明显优于对照组(P<0.05);两组患者治疗后的血流动力学及血液流变学指标较治疗前均有改善(P<0.05),且观察组优于对照组(P<0.05);观察组治疗后3个月、6个月的复发率均低于对照组(P<0.05)。结论:耳尖放血联合耳穴贴压辅助治疗后循环缺血性眩晕有良好疗效,可明显缓解后循环缺血性眩晕症状、改善脑供血、降低血液粘稠度,且复发率低,远期疗效较好。Objective:To observe the effect of auricular tip bloodletting combined with auricular plaster therapy on posterior circulation ischemic vertigo.Methods:A total of 120 patients with posterior circulation ischemic vertigo who were clinically confirmed and met the inclusion criteria were randomly divided into observation group and control group(n=60).The control group was given internal medicine symptomatic treatment,and the observation group was given auricular tip bloodletting combined with auricular plaster therapy on the basis of the control group.Two weeks of treatment is a cycle.The improvement of vertigo in the two groups after treatment was evaluated,and the hemodynamic changes and hemorheological changes of vertebral artery and basilar artery TCD before and after treatment were compared.The recurrence rate at 3 and 6 months after treatment was followed up.Results:Vertigo was improved in both groups after treatment,and the recovery rate and effective rate of observation group were significantly better than control group(P<0.05).The hemodynamics and hemorheology indexes of both groups were improved after treatment compared with before treatment(P<0.05),and the observation group was better than the control group(P<0.05).The recurrence rate of 3 and 6 months after treatment in the observation group was lower than that of the control group(P<0.05).Conclusion:Auricular tip bloodletting combined with auricular plaster therapy has a good effect in the treatment of posterior circulation ischemic vertigo,which can significantly relieve the symptoms of posterior circulation ischemic vertigo,improve cerebral blood supply,reduce blood viscosity,and has a low recurrence rate and good long-term efficacy.
