排卵障碍性异常子宫出血治疗选择:一项中国女性抽样横断面研究

摘　　要：目的对排卵障碍性异常子宫出血患者的治疗选择进行问卷调查,并尝试分析多种因素对选择结果的影响。方法采用问卷调查的方法,对来自全国493家医院、924位妇科医生门诊的2332例排卵障碍性子宫异常出血患者进行横断面调查。结果研究最终以2307例数据资料完整的患者作为分析样本,并按照年龄分为低年龄组(青春期14~18岁,39例)、中年龄组(性成熟期19~39岁,1939例)和高年龄组(绝经过渡期和绝经后期40~55岁,329例)。结果显示:各年龄组患者的主要症状均为月经不规律(43.6%~51.3%);赴医院就诊时,普通妇科的就诊率较高(79.9%~92.3%);在治疗方式中,以优思明/优思悦为代表的复方口服避孕药物在患者中接受度较高(35.9%~53.5%),可以满足大多数患者的治疗期望。结论排卵障碍性异常子宫出血患者的治疗意识积极良好;复方口服避孕药受到大多数患者的认可,有望成为治疗排卵障碍性异常子宫出血的重要方法。Objective:To investigate the selection for treatment of ovulatory dysfunctional abnormal uterine bleeding(AUB-O)by a questionnaire survey in Chinese women,and analyze the effects of various factors on the selection results.Methods:A cross-sectional survey was conducted among 2332 patients with AUB-O from 493 hospitals and 924 gynecologists in China.Results:A total of 2307 patients with complete data were included in the study.They divided into low age group(puberty:14-18 years old,n=39),middle age group(sexual maturity:19-39 years old,n=1939)and high age group(menopausal transition and postmenopausal period:40-55 years old,n=329).The results showed that irregular menstrual cycle was the main symptom in Chinese female AUB-O patients with different ages(43.6%-51.3%).Most of patients chose to seek help in the department of general gynecology(79.9%-92.3%).Among the treatment options,combined oral contraceptives(COCs),such as Yasmin,had a high acceptance in patients(35.9%-53.5%)and it can meet the treatment expectations of most patients.Conclusions:Among the patients with AUB-O,the treatment consciousness is positive and good.The combined oral contraceptive is recognized by most patients and is expected to become an important treatment for AUB-O.
