奶油制备CT造影剂观察消化道出血的实验研究

摘　　要：目的:以奶油为主要原料制备一种胃肠道CT低密度造影剂,并研究其对消化道出血的诊断价值。方法:以可食用淡奶油为原料,添加少量瓜尔豆胶,经搅拌机搅打后制得低密度造影剂。在CT扫描图像上观察其均匀性并测量CT值、比较其与血凝块的密度差异;在离体猪小肠中以混入肝素的血浆0.1、0.2、0.4mL模拟微量的消化道出血,观察该低密度造影剂对出血灶的显示效果,并比较其与中性对比剂(水)对出血的显示效果。结果:制得的低密度造影剂的CT扫描图像较均匀,CT值范围为-500~-700 Hu,与血凝块的密度差异明显;其能准确地显示和区分0.1、0.2、0.4 mL的出血灶,相对于中性对比剂(水)对出血的显示具有明显的优势。结论:以奶油添加瓜尔豆胶制备的胃肠道CT低密度造影剂均匀性良好,可以准确地显示出血灶,相对于中性对比剂(水)对消化道出血的诊断更为优越,值得进一步在临床诊断中试用。Objective:A low-density CT contrast agent for the gastrointestinal tract was prepared with cream as the primary raw material,and its diagnostic value for gastrointestinal bleeding was explored.Methods:The low-density contrast agent was prepared with edible whipped cream as raw material and mixed with a small amount of guar gum.After that,the homogeneity of the contrast agent was observed on the CT scan image,the CT value was measured,and its density difference between the blood clot was compared.In the isolated pig small intestine,0.1,0.2,and 0.4 mL of plasma mixed with heparin was used to simulate a small trace amount of gastrointestinal bleeding to observe the display effect of the low-density contrast agent on the bleeding focus and was compared with the neutral contrast agent(water).Results:The CT scan image of the prepared low-density contrast agent is relatively uniform,with a CT value ranging from-500 to-700 Hu,which was significantly different from the density of blood clots.The low-density contrast agent accurately displayed and distinguished 0.1,0.2,and 0.4 mL bleeding focus.Compared with the neutral contrast medium(water),it showed apparent advantages in the display of bleeding.Conclusion:The gastrointestinal CT low-density contrast agent prepared with cream and guar gum had good uniformity and can accurately display the bleeding focus.It is superior to the neutral contrast agent(water)for diagnosing gastrointestinal bleeding and is worth further clinical trials.
