疼痛链式管理方案在小儿腹腔镜术后镇痛中的应用效果分析

摘　　要：目的:分析探讨疼痛链式管理方案在小儿腹腔镜术后镇痛中的应用效果。方法:选取2020年1月—2020年12月于本院进行腹腔镜手术的86例腹股沟疝患儿作为研究对象,随机将其分为管理组与常规组,各43例。比较两组患儿手术后48小时的疼痛评分和陪护家长定义患儿疼痛控制的满意率及其控制疼痛过程中产生焦虑、抑郁等负面情绪。结果:术后48h内两组行为评估量表(FLACC)、面部表情疼痛量表(FPS)疼痛评分比较,管理组明显低于常规组(P<0.05);管理组陪护家长对疼痛控制的满意率为90.70%,明显高于常规组家长满意率的72.09%(P<0.05);管理组家长的医院焦虑、抑郁量表(HADS)评分均低于常规组家长(P<0.05)。结论:疼痛链式管理应用在小儿腹腔镜术后镇痛中,可有效减轻患儿术后疼痛感,缓解陪护家长焦虑、抑郁等负面情绪,提高家长对患儿术后镇痛的满意度。Objective:To analyze the application effect of pain chain management program in pediatric laparoscopic postoperative analgesia.Methods:A total of 86 children with inguinal hernia who underwent laparoscopic surgery in our hospital from January 2020 to December 2020 were selected as the research subjects,and they were randomly divided into a management group and a routine group,with 43 cases in each.The pain scores at 48 hours after the operation and the satisfaction rate of the children's pain control defined by the accompanying parents and the negative emotions such as anxiety and depression during pain control were compared between the two groups.Results:Comparing the pain scores of Behavior Assessment Scale(FLACC)and Facial Expression Pain Scale(FPS)between the two groups within 48 hours after operation,the management group was significantly lower than the routine group(P<0.05);the accompanying parents of the management group were satisfied with the pain control The rate of 90.70%was significantly higher than the parent satisfaction rate of 72.09%in the routine group(P<0.05).The Hospital Anxiety and Depression Scale(HADS)scores of the parents in the management group were lower than those in the routine group(P<0.05).Conclusion:The application of pain chain management in pediatric laparoscopic postoperative analgesia can effectively reduce postoperative pain in children,relieve anxiety,depression and other negative emotions of accompanying parents,and improve parents'satisfaction with postoperative analgesia.
