摘 要:目的:应用三维斑点追踪技术(3D-STE)评价青年男性力量型运动员左心房收缩功能。方法:选取青年男性力量型运动员(运动员组)和健康青年男性(对照组)各30例,应用常规超声心动图测量左房前后径(LAD)、心率(HR)及左室射血分数(LVEF);应用3D-STE测量左心房储蓄期长轴应变(LASr)及圆周应变(LASr_c),左心房导管期长轴应变(LAScd)及圆周应变(LAScd_c),左心房辅泵期长轴应变(LASct)、圆周应变(LASct_c)以及左心房最大容积(LAVmax),左心房最小容积(LAVmin)、左心房收缩前容积(LAVpre-A)、左心房容积指数(LAVI)、左心房排空率(LAEV)及左心房射血分数(LAEF),并计算左心房主动射血分数(LAaEF)及被动射血分数(LApEF)。分析以上两组数据之间的差异。结果:与对照组相比,运动员组HR、LAEF、LAaEF、LASr、LASr_c、LAScd、LAScd_c、LASct、LASct_c降低,LAD、LAVmax、LAVmin、LAVpre-A、LAVI、LAEV增加,差异有统计学意义(均P<0.05);LApEF降低,但差异无统计学意义(P>0.05)。结论:3D-STE可早期发现青年男性力量型运动员左心房收缩功能的亚临床改变。ObjectiveTo assess the left atrial systolic function in young male strength athletes by three-dimensional speckle tracking echocardiography(3D-STE).MethodsThirty young male strength athletes were selected into an athlete group and 30 healthy counterparts were chosen into a control group.The conventional echocardiography was applied to measure the left atrial diameter(LAD),heart rate(HR)and left ventricular ejection fraction(LVEF),while the 3D-STE was employed to measure the left atrial savings phase long-axis strain(LASr)and circumferential strain(LASr_c),left atrial conduit phase long-axis strain(LAScd)and circumferential strain(LAScd_c),left atrial co-pumping phase long-axis strain(LASct)and circumferential strain(LASct_c),as well as the left atrial maximum volume(LAVmax),minimum volume(LAVmin),presystolic volume(LAVpre-A),volume index(LAVI),emptying rate(LAEV)and ejection fraction(LAEF).The left atrial active ejection fraction(LAaEF)and passive ejection fraction(LApEF)were also calculated.The differences between the two groups were compared and analyzed.ResultsCompared with the control group,the average HR,LAEF,LASr,LASr_c,LAScd,LAScd_c,LASct,and LASct_c decreased significantly while the average LAD,LAVmax,LAVmin,LAVpre-A,LAVI and LAEV increased significantly in the athlete group(P<0.05 for all).The average LApEF of the athlete also decreased,but not significantly compared with the control group(P>0.05).Conclusion3D-STE can detect the subclinical changes of the left atrial systolic function in young male strength athletes at an early stage.
