加味参苓白术散联合常规治疗对脾虚挟瘀浊证持续性不卧床腹膜透析患者的临床疗效

摘　　要：目的考察加味参苓白术散联合常规治疗对脾虚挟瘀浊证持续性不卧床腹膜透析患者的临床疗效。方法66例患者随机分为对照组和观察组,每组33例,对照组给予常规治疗(纠正贫血、调控血压、维持机体物质代谢平衡等),观察组在对照组基础上加用加味参苓白术散,疗程16周。检测中医证候疗效、中医证候评分、TGF-β_(1)、α-SMA、E-cadherin、腹膜功能指标(UF、D/Pcr、D_(4)/D_(0)、Kt/V)变化。结果观察组中医证候总有效率高于对照组(P<0.05)。治疗后,观察组中医证候评分、TGF-β_(1)、α-SMA低于对照组(P<0.05),E-cadherin、UF、D_(4)/D_(0)、Kt/V更高(P<0.05)。结论加味参苓白术散联合常规治疗可降低脾虚挟瘀浊证持续性不卧床腹膜透析患者α-SMA、TGF-β_(1)水平,升高E-cadherin水平,防止腹膜纤维化,改善腹膜透析功能。AIM To investigate the clinical effects of Modified Shenling Baizhu Powder combined with conventional treatment on patients undergoing continuous ambulatory peritoneal dialysis due to Syndrome of Spleen Deficiency with Blood Stasis and Dampness Turbidity.METHODS Sixty-six patients were randomly assigned into control group(33 cases)for sixteen-week intervention of conventional treatment(correcting anemia,regulating blood pressure,maintaining the metabolic balance of body substances,etc.),and observation group(33 cases)for sixteen-week intervention of both Modified Shenling Baizhu Powder and conventional treatment.The changes in TCM symptom efficacy,TCM symptom score,TGF-β_(1),α-SMA,E-cadherin and peritoneal function indices(UF,D/Pcr,D_(4)/D_(0),Kt/V)were detected.RESULTS The observation group demonstrated higher total effective rate of TCM symptoms than the control group(P<0.05).After the treatment,the observation group displayed lower TCM symptom score,TGF-β_(1)andα-SMA than the control group(P<0.05),along with higher E-cadherin,UF,D_(4)/D_(0)and Kt/V(P<0.05).CONCLUSION For the patients undergoing continuous ambulatory peritoneal dialysis due to Syndrome of Spleen Deficiency with Blood Stasis and Dampness Turbidity,Modified Shenling Baizhu Powder combined with conventional treatment can reduceα-SMA and TGF-β_(1)levels,enhance E-cadherin level,prevent peritoneal fibrosis and improve peritoneal dialysis functions.
