摘 要:目的比较不同体脂率组肥胖患者CT肺动脉造影(CTPA)图像质量及辐射剂量,并分析肺动脉CT值、对比信噪比、噪声及有效剂量与体重、体重指数(BMI)、去脂体重(LBM)、体脂量及体脂率的相关性。方法根据体脂率分为肥胖(体脂率28%~34%)和超肥胖(体脂率≥35%),测量并比较2组患者的肺动脉CT值、噪声、CNR及辐射剂量,并分析肺动脉CT值、CNR、噪声及有效剂量与体重、BMI、LBM、体脂量及体脂率的相关性。结果不同体脂率组的肺动脉CT值、肺动静脉CT值比值、噪声及辐射剂量相比差异无统计学意义(P>0.05),SNR、CNR相比差异有统计学意义(P<0.05);体重、BMI、LBM、体脂量及体脂率与肺动脉平均CT值呈一般程度负相关;与有效剂量呈正相关,相关性较强,且相关系数均有统计学意义(P<0.05),受体重影响最大(r=0.783)。结论患者体脂率增加,会导致CTPA图像的SNR及CNR降低,患者所受辐射剂量增加;不同肥胖标准与图像质量相关性一般,与辐射剂量相关性较强,并受体重影响最大。Objective To compare the CTPA image quality and radiation dose of obese patients in different body fat percentage groups,and to analyze the correlation between pulmonary CT value,contrast signal-to-noise ratio,noise and effective dose and body weight,body mass index(BMI),lean body weight(LBM),body fat amount and body fat percentage.Methods The patients were divided into obesity(body fat percentage 28%-34%)and super obesity(body fat percentage≥35%)according to body fat percentage.The differences of pulmonary CT value,noise,comparison noise ratio(CNR)and radiation dose were measured and compared between two groups,and the correlations of pulmonary CT value,CNR,noise and effective dose with body weight,BMI,LBM,body fat amount and body fat percentage were analyzed.Results There were no statistically significant differences in pulmonary CT value,the ratio of pulmonary arterio-venous CT value,noise and radiation dose between different body fat percentage groups(P>0.05),SNR and CNR were significantly different(P<0.05);Body weight,BMI,LBM,body fat amount and body fat percentage were generally negatively correlated with mean pulmonary CT value;there were positively correlated with effective dose(P<0.05),which was most affected by body weight(r=0.783).Conclusions The SNR and CNR of CTPA images are decreased and the radiation dose of patients is increased with the increase of body fat percentage.The correlation between different obesity criteria and image quality is normal,the correlation between different obesity criteria and ED is strong,and the influence of weight is the biggest.
