腹直肌旁入路与髂腹股沟入路治疗髋臼双柱骨折的体会

摘　　要：目的探讨腹直肌旁入路与髂腹股沟入路在治疗髋臼双柱骨折的临床疗效。方法回顾性分析采用髂腹股沟入路治疗的髋臼双柱骨折21例(A组)、采用腹直肌旁入路治疗的髋臼双柱骨折17例(B组)患者资料,记录2组手术时间、术中出血量及术后并发症。术后按照Matta标准评价骨折复位情况等。结果A组7例解剖复位,11例复位满意,3例复位不满意,末次随访髋关节功能评分为(15.81±1.44)分。B组4例解剖复位,10例复位满意,3例复位不满意,末次随访髋关节功能评分为(15.30±1.57)分。A、B 2组在手术时间、术中出血量、术后引流量、术后并发症、骨折复位、髋关节功能评分上差异均无统计学意义,仅在手术切口长度方面差异有统计学意义(P<0.05)。结论腹直肌旁入路和髂腹股沟入路在治疗髋臼双柱骨折临床应用中,均能获得满意疗效,二者无明显优劣之别。Objective To compare the clinical effects of rectus abdominis approach and ilioinguinal approach in the treatment of double-column acetabular fractures.Methods The clinical data of 21 patients with double column acetabular fractures treated by ilioinguinal approach(group A)and 17 patients with double column acetabular fractures treated by rectus abdominis approach(group B)were analyzed retrospectively.The operation time,blood loss,complications and whether combined with posterior operation were recorded in the two groups,and the fracture reduction was evaluated according to Matta standard.Patients in both groups were followed up.Results In group A,the reduction was evaluated by Matta:anatomical reduction was achieved in 7 cases,reduction was satisfactory in 11 cases and unsatisfactory in 3 cases.The hip function score at the last follow-up was(15.81±1.44).In group B,the reduction was evaluated by Matta:anatomical reduction in 4 cases,satisfactory reduction in 10 cases and unsatisfactory reduction in 3 cases.The hip function score of the last follow-up was(15.30±1.57).The results showed that there was no significant difference in operation time,blood loss,postoperative drainage,complications,fracture reduction and hip function score between group A and group B,and there was only in the length of surgical incision(P<0.05).Conclusion Both rectus abdominis approach and ilioinguinal approach can achieve satisfactory results in the treatment of double-column acetabular fractures.
