彩色多普勒超声对钝性腹部创伤致内脏破裂出血的诊断价值分析

摘　　要：目的:探讨彩色多普勒超声对钝性腹部创伤(BAT)致内脏破裂出血的诊断价值。方法:选择2020年2月-2021年12月就诊于某院超声科的98例BAT患者,均接受彩色多普勒超声检查。以随访或手术结果为金标准,分析彩色多普勒超声对BAT致内脏破裂出血检出率。结果:98例患者经随访或手术证实共存在脏器破裂出血118个,其中脾破裂58个(49.15%),肝破裂35个(29.66%),胰腺破裂2个(1.69%),肾破裂15个(12.71%),膀胱破裂2个(1.69%),小肠破裂6个(5.08%);经彩色多普勒超声检查显示BAT致内脏破裂出血96个(81.36%),其中检出率最高的为膀胱破裂,为100.00%,其次为肝破裂,检出率为91.43%,检出率最低的为小肠破裂,均未被检出,漏诊9个(7.63%),未明确具体脏器破裂13个(11.02%),检查提示为盆腹腔积液。结论:彩色多普勒超声可准确诊断BAT致内脏破裂出血,为临床治疗提供参考。Objective:To investigate the diagnostic value of color Doppler ultrasound in visceral rupture and hemorrhage caused by blunt abdominal trauma(BAT).Methods:A total of 98 patients with BAT who were admitted to a hospital from February 2020 to December 2021 were selected and received color Doppler ultrasound examination.Taking follow-up or surgical results as the gold standard,the detection rate of BAT-induced visceral rupture and hemorrhage by color Doppler ultrasound was analyzed.Results:A total of 118 visceral ruptures were confirmed by follow-up or surgery in 98 patients,including 58 ruptures of the spleen(49.15%),35 ruptures of the liver(29.66%),2 ruptures of the pancreas(1.69%),and 15 ruptures of the kidneys.(12.71%),2 bladder ruptures(1.69%),and 6 small bowel ruptures(5.08%);Color Doppler ultrasound showed 96(81.36%)visceral ruptures and hemorrhage caused by BAT,of which the highest detection rate was Bladder rupture was 100.00%,followed by liver rupture with the detection rate of 91.43%.The lowest detection rate was small bowel rupture,none of which was detected,9 cases were missed(7.63%),and 13 cases were unspecified organ ruptures(11.02%),the examination suggested pelvic ascites.Conclusion:Color Doppler ultrasound can accurately diagnose visceral rupture and hemorrhage caused by BAT,and provide reference for clinical treatment.
