超声造影特征联合TIC参数对鼻咽癌颈部淋巴结转移的诊断价值

摘　　要：目的探讨超声造影(CEUS)定性特征联合时间-强度曲线(TIC)定量参数对鼻咽癌(NPC)颈部淋巴结转移的诊断价值。方法回顾性分析78例NPC患者共计103枚颈部增大淋巴结,依据病理结果分为转移组(56枚)、未转移组(47枚),比较两组淋巴结的CEUS特征、TIC参数,绘制ROC曲线评价CEUS特征联合TIC参数对NPC颈部淋巴结转移的诊断效能。结果NPC转移组淋巴结CEUS以向心性或混合性灌注、不均匀增强为主;与未转移组相比,转移组淋巴结增强强度更低,更易有灌注缺损;转移组淋巴结的始增时间、达峰时间、峰值减半时间、峰值强度及曲线下面积均小于未转移组;以上差异均有统计学意义(P<0.05)。CEUS特征联合TIC参数诊断NPC颈部淋巴结转移的灵敏度为91.1%、特异度为91.5%、曲线下面积为0.948。结论CEUS特征联合TIC参数在诊断NPC颈部淋巴结转移中有一定的应用价值。Objective To investigate the value of Contrast-enhanced ultrasound(CEUS)features combined with time intensity curve(TIC)parameters in diagnosis of cervical lymph node metastasis of nasopharyngeal carcinoma(NPC).Methods A total of 103 cervical swollen lymph nodes from 78 NPC patients were retrospectively analyzed.They were divided into metastatic group(56)and non-metastatic group(47)according to the pathological results.And CEUS features and TIC parameters of the two groups were compared.Draw ROC curves to evaluate the diagnostic efficacy of CEUS features combined with TIC parameters in diagnosis of cervical lymph node metastasis of NPC.Results Compared with non-metastatic group,the metastatic group was mainly manifested as centripetal or mixed perfusion,inhomogeneous enhancement,lower enhancement intensity and more likely to have perfusion defect(P<0.05).And the rise time,time to peak,peak intensity,time from peak to one half,area under curve of the metastatic group were lower than those of the non-metastatic group(P<0.05).The sensitivity,specificity and area under curve of CEUS features combined with TIC parameters in diagnosing NPC cervical lymph node metastasis were 91.1%,91.5%and 0.948 respectively.Conclusions CEUS features combined with TIC parameters have certain application value in the diagnosis of cervical lymph node metastasis of NPC.
