摘  要:目的研究瞬时弹性成像技术(TE)在脑死亡器官捐献供肝质量评估的价值。方法选取40例脑死亡器官捐献者,采用FibroTouch检测其肝纤维化及脂肪变性程度,以病理为金标准,比较肝纤维化及脂肪变性各组的肝脏硬度(LS)及受控衰减参数(CAP),计算灵敏度、特异度、临界值,并绘制ROC曲线。结果肝纤维化F2期的LS值与F0及F1期差异有统计学意义。LS诊断供肝F2期纤维化的AUROC为0.871,临界值为10.65 kPa。肝脂肪变性各组CAP值差异有统计学意义,CAP诊断供肝S1及S2期脂肪变性的AUROC为0.878、0.900,临界值为248.5 dB/m和278 dB/m。结论瞬时弹性成像技术能够客观评价脑死亡捐献供体肝纤维化及脂肪变性的程度,具有较高的诊断价值。Objective This study aimed to evaluate the value of transient elastography(TE)to assess liver grafts from brain dead donors.Methods Liver grafts from 40 brain-dead donors from our hospital were selected,and the degree of liver fibrosis and steatosis were examined by FibroTouch.Taking pathological as the gold standard,the LS and CAP value of fibrosis and steatosis groups were compared,the sensitivity,specificity,and cut-off value were calculated by ROC analysis.Results The LS values in different fibrosis groups were statistically significant.The AUROC was 0.871 in F2 stage fibrosis and the cut-off value was 10.65 kPa.There were statistically significant differences in CAP values in steatosis groups.The AUROC of S1 and S2 stage were 0.878 and 0.900,with cut-off values 248.5 dB/m and 278 dB/m,respectively.Conclusions Transient elastography could evaluate the degree of liver fibrosis and steatosis in brain-dead donors with a high diagnostic accuracy.
