摘 要:目的对比研究立位肢体挤压法和卧位Valsalva法在超声检测下肢小腿CockettⅠ穿静脉瓣膜功能不全中的准确性。方法纳入临床初步怀疑为下肢穿静脉功能不全的患者共33例,分别进行了立位肢体近端挤压法和卧位Valsalva诱导法超声检测,并与X线静脉顺行性造影相对照,比较和分析两种方法在检测CockettⅠ穿静脉瓣膜功能不全的结果。结果卧位Valsalva诱导法和立位肢体挤压法彩色多普勒超声检测出功能不全的穿静脉分别为55条和76条,与X线静脉顺行性造影对比的符合率分别为69.5%和96.2%;两者对比具有明显的差异(P<0.001)。结论在超声检测Cockett I穿静脉功能不全中,采用立位肢体挤压法较卧位Valsalva诱导法的检出率高。Objective To compare the accuracy of standing limb squeeze method and supine position Valsalva method in ultrasonic detection of CockettⅠperforating valvular insufficiency of lower limbs.Methods A total of 33 patients who were initially suspected of lower extremity perforating venous insufficiency in our hospital were enrolled.They underwent the ultrasound examination of standing limb squeeze method and supine position Valsalva induction method.The results of the two methods in detecting CockettⅠperforating valve insufficiency were analyzed and compared with X-ray venous anterograde contrast.Results There were 55 and 76 defective perforating veins detected by the Valsalva induction method in the supine position and the color Doppler ultrasound in the standing limb squeeze method respectively,and the coincidence rates compared with X-ray venous antegrade contrast were 69.5%and 96.2%.There was a significant difference between the two methods(P<0.001).Conclusions In the ultrasonic detection of Cockett I perforating venous insufficiency,the detection rate of the standing limb squeeze method is higher than that of the supine Valsalva induction method.
