摘 要:目的探讨超声造影(CEUS)定量分析评估二乙基亚硝胺(DEN)诱发性大鼠原位肝癌(HCC)模型肿瘤血管生成的应用价值。方法20只雄性Wistar大鼠按30mg/kg腹腔注射DEN,定期超声检查发现肝脏结节的大鼠进行CEUS检查和定量分析。检查后处死取标本进行病理诊断,软件自动检测肿瘤和肝实质CD31和α-SMA免疫染色阳性像素百分比。比较肝肿瘤和肝实质CEUS定量参数、病理免疫染色的差异,分析肝肿瘤CEUS定量参数和病理免疫染色的相关性。结果HCC的强度增幅(△PI)、上升斜率(S_(wash-in))、达峰时间(TTP)和平均渡越时间(MTT)和肝实质有统计学意义(P<0.05)。HCC的CD31和α-SMA阳性表达显著高于肝实质(P<0.05)。HCC的△PI和CD31阳性表达有显著相关性(r=0.589,P=0.008)。结论CEUS定量分析能在一定程度上反映诱发性大鼠肝癌模型肿瘤血管生成。Objective To explore the application value of quantitative analysis of contrast-enhanced ultrasound(CEUS)for angiogenesis in a diethylnitrosamine(DEN)-induced hepatocellular carcinoma(HCC)rat model.Methods Twenty male Wistar rats were injected DEN at a dose of 30 mg/kg body weight.The rats underwent CEUS examination and quantitative analysis when the hepatic nodules were found by B-mode ultrasound.The hepatic nodules were confirmed by histologically.The area positive for CD31 andα-SMA of the tumor area and the liver parenchyma was measured automatically.The differences of CEUS quantitative parameters and immunostaining between the tumors and the liver parenchyma were compared.The correlation between CEUS quantitative parameters and immunostaining of tumors was analyzed.Results△PI,S_(wash-in),TTP and MTT of HCC were significantly different from those of the liver parenchyma(P<0.05).The area positive for CD31 andα-SMA of the tumor area was higher than that of the liver parenchyma(P<0.05).There was a significant association between△PI and the area positive for CD31 of the tumor area(r=0.589,P=0.008).Conclusions Quantitative analysis of dynamic CEUS could reflect tumor angiogenesis of the DEN-induced HCC rat model.
