摘　　要：目的探讨腮腺乳腺样分泌性癌(MASC)的超声表现及其诊断价值。方法回顾性分析5例病理证实为MASC患者的临床、超声及病理资料,总结MASC的超声表现。结果根据内部回声分为囊实性及囊性两型,囊实性4例(4/5),囊性1例(1/5);囊实性型中3例囊性为主(3/4),1例实性为主(1/4),囊性为主3例均可见部分实性成分呈乳头状突起;5例均为单发病灶,结节边界清晰(5/5);1例形态规则(1/5),4例形态欠规则(4/5);后方回声均增强(5/5);血流信号为0~Ⅰ级,0级1例(1/5),Ⅰ级4例(4/5)。结论MASC可能具备特征性的超声表现,即腮腺内单发的囊性为主的囊实性结节,实性成分呈乳头状突起。当肿瘤呈实性为主或囊性时,声像图缺乏特征性,其诊断仍需依靠病理检查。Objective To investigate the ultrasound manifestations and diagnostic value of mammary analogue secretory carcinoma(MASC)of the parotid gland.Methods The clinical,ultrasonic and pathological data of five patients with pathologically confirmed MASC were retrospectively analyzed,and the ultrasonic manifestations of MASC were summarized.Results According to internal echo,five patients were divided into two types:cystic-solid type and cystic type.Four cases were cystic-solid type(4/5),one case was cystic type(1/5).Three cases of cystic-solid type were predominantly cystic(3/4),and solid components were seen as papillary protrusions.One case was predominantly solid(1/4).Five cases of nodules were all solitary and have clear boundaries(5/5),1 case was regular(1/5),four cases were irregular(4/5).Posterior acoustic enhancement was present in all cases(5/5).The blood flow signal was 0-Ⅰgrade with one case of 0 grade(1/5)and four cases of I grade(4/5).Conclusions The ultrasonic characteristic performance of MASC lesions may be the solitary cystic-solid nodules,which were predominantly cystic accompanied by solid components with papillary protrusions.However,when the ultrasonic image is predominantly solid or cystic type,and the final diagnosis of MASC still depended on pathological examination because of lack of specificity.
