脾淋巴管瘤的超声诊断

摘　　要：目的探讨脾淋巴管瘤的超声声像图特征及超声诊断临床价值。方法对7例经手术证实为脾淋巴管瘤的二维超声、彩色多普勒超声及3例患者的超声造影声像图进行回顾性分析。结果7例脾淋巴管瘤中5例超声诊断正确,2例误诊为血管瘤。有5例病理诊断为囊性淋巴管瘤,其超声声像图较典型,二维超声表现为囊性无回声区伴或不伴回声带分隔,彩色多普勒超声囊壁及回声带分隔上可见血流信号,超声造影囊壁包膜及回声带分隔见造影剂灌注,余囊腔呈无增强。另2例病理诊断为海绵状淋巴管瘤,常规超声表现为中等回声及稍高回声,周边及内部见少许血流信号,未行超声造影检查,均误诊为血管瘤。结论囊性脾淋巴管瘤具有较典型超声声像图特征,对术前临床诊断脾淋巴管瘤具有较高参考价值。Objective To investigate the ultrasonographic features and the clinical value of ultrasonography in the diagnosis of splenic lymphangioma.Methods Features of two-dimensional and color Doppler ultrasonography in seven cases of splenic lymphangioma confirmed by operation were analyzed retrospectively,and additional analyze of contrast-enhanced ultrasonography were performed in 3 cases.Results Among the 7 cases of splenic lymphangioma,5 cases were correctly diagnosed,2 cases were misdiagnosed as hemangioma.5 cases were diagnosed as cystic lymphangioma by histopathology.The ultrasonic image was typical.The two-dimensional ultrasound showed cystic anechoic area with or without septation.Blood flow signals were seen on the cystic wall and septation by color Doppler ultrasound.Contrast agent perfusion was seen on the cystic wall and septation by contrast-enhanced ultrasound,and the remaining cystic cavity showed no enhancement.Two cases were diagnosed as cavernous lymphangioma.Conventional ultrasound showed isoecho and slightly hyperecho.A few blood flow signals in periphery and inside.These two cases were misdiagnosed as hemangioma without contrast-enhanced ultrasonography.Conclusions According to the typical features of conventional ultrasound and contrast-enhanced ultrasound,it has a highly clinical value in the diagnosis of splenic lymphangioma before operation.
