疏肝理脾汤治疗腹泻型肠易激综合征肝郁脾虚证临床观察

摘　　要：目的观察疏肝理脾汤治疗腹泻型肠易激综合征肝郁脾虚证患者的临床疗效。方法将2018年1月-2019年6月于北京中医药大学东直门医院就诊的94例腹泻型肠易激综合征肝郁脾虚证患者随机分为治疗组48例和对照组46例,治疗组应用中药疏肝理脾汤治疗,对照组口服马来酸曲美布汀胶囊治疗,观察2组治疗期间的不良反应及治疗前后的症状变化,比较2组的症状评分和中医证候疗效;治疗结束4周后进行随访,比较2组的复发率。结果对照组不良反应发生率4.35%(2/46),治疗组未出现不良反应(P>0.05);与治疗前相比,治疗后2组症状评分均明显下降(P<0.01),且治疗组低于对照组(P<0.01);治疗组中医证候总有效率高于对照组(P<0.05);治疗组复发率低于对照组(P<0.05)。结论疏肝理脾汤可显著改善腹泻型肠易激综合征肝郁脾虚证患者的临床症状,具有不良反应少、复发率低的优势。Objective To observe the clinical effect of Shugan Lipi decoction on patients with diarrhea irritable bowel syndrome with liver depression and spleen deficiency.Methods From January 2018 to June 2019,94 patients in Tongzhou branch,Dongzhimen hospital with diarrhea irritable bowel syndrome of liver depression and spleen deficiency were randomly divided into treatment group of 48 cases and control group of 46 cases.The treatment group was treated with Shugan Lipi decoction,and the control group was treated with trimebutine maleate capsules.Adverse reactions during treatment and symptom score before and after treatment were observed.The symptom efficacy and traditional Chinese medicine syndrome efficacy of the two groups were evaluated.Follow-up was conducted 4 weeks after treatment,and the recurrence rate between the two groups was compared.Results The incidence of adverse reactions in control group was 4.35%(2/46),there was no adverse reaction in the treatment group(P>0.05).Compared with before treatment,the symptom scores of both groups decreased significantly after treatment(P<0.01),and the symptom score of the treatment group was lower than that of the control group(P<0.01).The total effective rate of traditional Chinese medicine syndrome in treatment group was higher than that in control group(P<0.05).The recurrence rate of the treatment group was lower than that of the control group(P<0.05).Conclusion Shugan Lipi decoction can significantly improve the clinical symptoms of diarrhea irritable bowel syndrome with liver depression and spleen deficiency,and it has the advantages of less adverse reactions and low recurrence rate.
