超声监测下全麻手法松解结合银质针温针灸治疗重度肩周炎的临床观察

摘　　要：目的观察全麻下手法松解结合银质针温针灸治疗重度肩周炎的临床疗效和安全性。方法纳入2016年6月-2018年12月上海市光华医院收治的重度肩周炎患者90例,采用随机数字表法分为治疗组和对照组,各45例。治疗组采用超声监测下全麻手法松解+银质针温针灸治疗;对照组采用普通毫针结合银质针温针灸治疗。评估2组治疗前后肩周炎疗效评分的改变以及治疗的安全性。结果治疗前,2组的肩周炎疗效评定量表各指标评分比较差异均无统计学意义(P>0.05);治疗后,治疗组肩周炎疗效评定量表各指标的评分均高于对照组,差异有统计学意义(P<0.05)。治疗组松解过程中超声监测下未发现肌腱韧带撕裂损伤。2组患者在治疗过程中及治疗后均未发生不良反应。结论超声监测下全麻手法松解结合银质针温针灸治疗重度肩周炎可显著改善患者肩关节功能,且安全性较高。Objective To observe the clinical efficacy and safety of generalized anesthesia manipulative release combined with silver needles warm acupuncture in the treatment of severe periarthritis of shoulder.Methods From June 2016 to December 2018,90 patients in Shanghai Guanghua hospital of integrated traditional Chinese and Western medicine with severe frozen shoulder were enrolled and randomly divided into a treatment group and a control group,with 45 cases in each group.The treatment group was treated with general anesthesia manual release under ultrasound monitoring combined with silver needle warming acupuncture.The control group was treated with ordinary needles and silver needles warming acupuncture.The changes of the frozen shoulder efficacy score,the safety of treatment were observed and recorded before treatment and after treatment in the two groups.Results Before treatment,there was no statistical significance in the scores of the efficacy rating scale for periarthritis of shoulder between the two groups(P>0.05),and the improvement of the efficacy rating scale for periarthritis of shoulder in the treatment group was better than that in the control group after treatment,with statistical significance(P<0.05).No tendon and ligament tears were found in the treatment group under ultrasound monitoring during the release process.There were no adverse reactions in 2 groups during and after treatment.Conclusion Ultrasound monitoring suggests that manual release combined with silver needle warming acupuncture for severe periarthritis of shoulder can significantly improve shoulder function and has high safety.
