摘　　要：目的比较衰弱表型、临床衰弱量表、FRAIL量表以及中文版Tilburg衰弱指标在老年血液透析患者中的评估效果,寻找适合老年血液透析患者的衰弱评估工具。方法选取2019年7月—2019年12月在长江大学附属第一医院血液净化中心进行维持性血液透析的老年患者252名,采用四种衰弱量表进行衰弱评估,比较评估结果。结果四种衰弱量表的衰弱检出率分别为44.5%(衰弱表型)、52.8%(临床衰弱量表)、39.3%(FRAIL量表)和26.6%(中文版Tilburg衰弱指标),组间差异有统计学意义(P<0.05);两两比较结果显示:衰弱表型与临床衰弱量表、FRAIL量表评估结果的比较,差异无统计学意义(P>0.05);其余评估工具评估结果的两两比较,差异均有统计学差异(P<0.05)。结论四种衰弱评估工具的评估结果不完全一致,衰弱表型是更适合老年血液透析患者的衰弱评估工具。Objective To compare the effects of frailty phenotype,clinical frailty Scale,FRAIL Scale,and Chinese version Tilburg frailty indicator on elderly patients on hemodialysis,and to search for a frailty assessment tool suitable for elderly patients on hemodialysis.Methods A total of 252 elderly patients who underwent maintenance hemodialysis in the Blood Purification Center of the First Affiliated Hospital of Yangtze University from July 2019 to December 2019 were selected to evaluate frailty using four frailty scales and compare the evaluation results.Results The frailty detection rates of the four frailty scales were 44.5%(frailty phenotype),52.8%(clinical frailty scale),39.3%(FRAIL scale)and 26.6%(Chinese Tilburg frailty indicator scale),respectively,with statistically significant differences among groups(P<0.05).Pairwise comparison showed that there was no statistical significance in the comparison of frailty phenotype with the clinical frailty scale and FRAIL scale(P>0.05).Pairwise comparison of the results of other assessment tools showed statistical differences(P<0.05).Conclusion The results of four frailty assessment tools are not completely consistent,frailty phenotype is more suitable assessment tool for elderly patients with hemodialysis.
