变应性鼻炎患者血清MCP-1、IFN-γ表达与血清总免疫球蛋白E的关系

摘　　要：目的探讨变应性鼻炎患者血清单核细胞趋化蛋白-1(MCP-1)、干扰素-γ(IFN-γ)表达与血清总免疫球蛋白E(IgE)的关系。方法选取2019年1月—2021年3月变应性鼻炎患者126例(病例组),根据患者病情严重程度分为轻度组56例和中重度组70例。选取无变应性鼻炎的健康志愿者63例(对照组)。检测血清MCP-1、IFN-γ、总IgE水平,分析相关性,并绘制ROC曲线分析MCP-1、IFN-γ对变应性鼻炎的诊断效能。结果病例组血清MCP-1水平高于对照组,IFN-γ水平低于对照组,总IgE水平高于对照组(均P<0.05)。变应性鼻炎中重度组血清MCP-1水平高于轻度组,IFN-γ水平低于轻度组,总IgE水平高于轻度组(均P<0.05)。变应性鼻炎患者血清MCP-1水平与血清总IgE水平呈正相关(r=0.533),与病情严重程度呈正相关(r=0.420)。血清IFN-γ水平与血清总IgE水平呈负相关(r=-0.629),与病情严重程度呈负相关(r=-0.473)。血清MCP-1诊断变应性鼻炎的灵敏度为82.54%,特异度为71.43%(AUC=0.808,95%CI:0.743~0.872)。血清IFN-γ诊断变应性鼻炎的灵敏度为73.02%,特异度为63.49%(AUC=0.718,95%CI:0.641~0.796)。结论变应性鼻炎患者血清MCP-1表达升高、IFN-γ表达降低,与血清总IgE水平密切相关,对变应性鼻炎有一定的诊断价值。Objective To investigate the relationship between the expression of serum monocyte chemoattractant protein-1(MCP-1)and interferon-γ(IFN-γ)and serum total immunoglobulin E(IgE)in patients with allergic rhinitis.Methods A total of one hundred and twenty-six patients with allergic rhinitis(case group)were enrolled in this study between January 2019 and March 2021.According to the severity of the disease,the patients were divided into mild group(56 cases)and moderate to severe group(70 cases).Meanwhile,63 healthy volunteers were selected as the control group.Serum MCP-1,IFN-γand total IgE levels were detected,and their correlation was analyzed.The ROC curve was used to analyze the diagnostic efficiencies of MCP-1 and IFN-γfor allergic rhinitis.Results Serum MCP-1 and total IgE levels in the case group were higher than those in the control group,and IFN-γlevel was lower than that in the control group(P<0.05).SerumMCP-1 and total IgE levels in the moderate to severe group were higher than those in the mild group,and IFN-γlevel was lower than that in the mild group(P<0.05).Serum MCP-1 level was positively correlated with serum total IgE level(r=0.533),and the severity of the disease(r=0.420).Serum IFN-γlevel was negatively correlated with serum total IgE level(r=-0.629),and the severity of the disease(r=-0.473).The sensitivity of serum MCP-1 in the diagnosis of allergic rhinitis was 82.54%,and the specificity was 71.43%(AUC=0.808,95%CI:0.743-0.872).The sensitivity of serum IFN-γin the diagnosis of allergic rhinitis was 73.02%,and the specificity was 63.49%(AUC=0.718,95%CI:0.641-0.796).Conclusion The expression of serum MCP-1 is elevated and the expression of IFN-γis reduced in patients with allergic rhinitis.The two are closely related to serum total IgE level and have certain diagnostic value for allergic rhinitis.
