UAE联合宫腔镜治疗在CSP患者治疗中的效果及其安全性分析

摘　　要：目的探讨子宫动脉栓塞术(uterine artery embolization,UAE)联合宫腔镜治疗剖宫产瘢痕部位妊娠(cesarean scar pregnancy,CSP)的效果及安全性。方法选取2018年1月—2020年7月在该院治疗的CSP患者106例进行回顾性分析,根据采取的治疗方案分为观察组(n=59)和对照组(n=47),其中观察组给予UAE联合宫腔镜治疗,对照组给予宫腔镜治疗,观察两组手术时间、术中出血量等,检测手术前后血人绒毛膜促性激素(β-human chorionic gonadotropin,β-HCG)水平、月经恢复时间、月经恢复情况。结果观察组术中出血量为(87.80±21.54)ml,明显少于对照组(P<0.05);观察组和对照组手术时间和术后住院时间差异比较无统计学意义(P>0.05);观察组治疗后1周血β-HCG水平为(451.18±78.84)U/L,明显低于对照组(P<0.05),以上差异均有统计学意义;观察组和对照组治疗后月经恢复时间、月经量、月经持续时间、月经第2天查FSH、LH和E2比较差异无统计学意义(P>0.05);观察组和对照组总并发症差异比较无统计学意义(P>0.05)。结论UAE联合宫腔镜治疗CSP有较好的效果,具有术中出血量少,术后血β-HCG水平明显降低,未发现对卵巢功能有明显影响等优点,值得临床使用。Objective To investigate the efficacy and safety of uterine artery embolization(UAE)combined with hysteroscopy in the treatment of cesarean scar pregnancy(CSP).Methods One hundred and six CSP patients who were treated in our hospital from January 2018 to July 2020 were divided into observation group(n=59)and control group(n=47)according to the treatment plan,the observation group was treated with UAE combined with hysteroscopy,and the control group was treated with hysteroscopy,the time of operation and the amount of bleeding and so on in the two groups were observed,the human chorionic gonadotropin(β-HCG),menstrual recovery time,menstrual recovery were measured before and after operation.Results The intraoperative blood loss in the observation group was(87.80±21.54)ml,which was significantly lower than that in the control group(P<0.05),the above differences are statistically significant;There were no significant difference between the observation group and the control group in operation time and postoperative hospital stay(P>0.05);The bloodβ-HCG level in the observation group at one week after treatment were(451.18±78.84)U/L,which were significantly lower than those in the control group(P<0.05);There were no significant difference in menstrual recovery time,menstrual volume,menstrual duration,FSH,LH and E2 between the observation group and the control group at 1 month after treatment(P>0.05);There was no significant difference in total complications between the observation group and the control group(P<0.05).Conclusion UAE combined with hysteroscopy for CSP has a good effect,has the advantages of less intraoperative blood loss,significantly reduced l bloodβ-HCG level after operation,No obvious effect on ovarian function is found,and is worthy of clinical use.
