血清阿立新-A、葡萄糖转运蛋白-4与多囊卵巢综合征不孕症患者促排卵疗效的相关性

摘　　要：目的分析血清阿立新-A(Orexin-A)、葡萄糖转运蛋白-4(GLUT-4)与多囊卵巢综合征(PCOS)不孕症患者促排卵疗效的相关性。方法选取新郑市天佑中医院2020年1月至2021年3月诊治的73例PCOS不孕症患者,将其纳入研究组。另选择同期40例健康体检并证实为健康者(女性)作为对照组。抽取患者外周血检测Orexin-A、GLUT-4、血清黄体生成素(LH)、卵泡刺激素(FSH)、雌二醇(E_(2))水平;检测研究组患者胰岛素抵抗指数并评定促排卵疗效。根据胰岛素抵抗指数将患者分成抵抗组及非抵抗组;根据促排卵情况将患者分成成功排卵组及失败组。结果研究组血清Orexin-A、GLUT-4水平均低于对照组(P<0.05)。胰岛素抵抗组血清Orexin-A、GLUT-4水平均低于非抵抗组(P<0.05)。成功排卵组血清Orexin-A、GLUT-4水平均高于失败组(P<0.05)。研究组LH、E_(2)水平均低于对照组(P<0.05),FSH水平与对照组比较差异无统计学意义(P>0.05)。胰岛素抵抗组LH水平高于非抵抗组(P<0.05),E_(2)水平低于非抵抗组(P<0.05),FSH水平与非抵抗组比较差异无统计学意义(P>0.05)。成功排卵组LH、E_(2)水平高于失败组(P<0.05),FSH水平与失败组比较差异无统计学意义(P>0.05)。血清Orexin-A、GLUT-4水平与LH呈正相关(P<0.05),与E_(2)、FSH无相关性(P>0.05)。血清Orexin-A与胰岛素抵抗指数无相关性(P>0.05),GLUT-4水平与胰岛素抵抗指数呈负相关(P<0.05),血清Orexin-A、GLUT-4与排卵疗效呈正相关(P<0.05)。结论PCOS不孕症患者血清Orexin-A、GLUT-4、LH、E_(2)水平降低,血清Orexin-A、GLUT-4水平与LH、排卵疗效有关,GLUT-4水平与胰岛素抵抗指数呈负相关。Objective To investigate the correlation of serum Orexin-A and glucose transporter type 4(GLUT4)with the efficacy of ovulation induction in patients with polycystic ovary syndrome(PCOS)infertility.Methods Seventy-three women with PCOS infertility in Xinzheng Tianyou Hospital from January 2020 to March 2021 were enrolled as study group,meantime,another 40 healthy individuals underwent physical examination were selected as control group.The levels of Orexin-A,GLUT-4,luteinizing hormone(LH),follicle stimulating hormone(FSH)and estradiol(E_(2))were detected in peripheral blood.The insulin resistance index of the patients in the study group was detected and the efficacy of ovulation induction was evaluated.According to insulin resistance index,patients were divided into resistance group and non-resistance group,according to ovulation induction,patients were divided into successful ovulation group and failure group.Results Serum levels of Orexin-A and GLUT-4 in study group were lower than those in control group(P<0.05).Serum levels of Orexin-A and GLUT-4 in insulin resistance group were lower than those in non-resistance group(P<0.05).Serum levels of Orexin-A and GLUT-4 in successful ovulation group were lower than those in failure group(P<0.05).The levels of LH and E_(2) in study group were lower than those in control group(P<0.05),while there was no statistical difference in the level of FSH compared with the control group(P>0.05).The LH level in the insulin resistance group was higher than that in the non-resistance group(P<0.05),the E_(2) level was lower than that in the non-resistance group(P<0.05),and there was no statistical difference between the FSH level and the non-resistance group(P>0.05).The levels of LH and E_(2) in the successful ovulation group were higher than those in the failure group(P<0.05),and there was no statistical difference in the level of FSH compared with the failure group(P>0.05).Serum levels of Orexin-A and GLUT-4 were positively correlated with LH(P<0.05),but had no correlation with E_(2
