2型糖尿病患者糖化血红蛋白、超氧化物歧化酶水平与角膜神经损伤的相关性

摘　　要：目的探讨糖化血红蛋白、超氧化物歧化酶在2型糖尿病(T2DM)患者中的表达以及其与角膜神经损伤的相关性。方法选取焦作市第二人民医院2019年12月至2021年12月收治的80例T2DM患者为研究组,选取同期95例健康体检人群作为对照组。检测受试者糖化血红蛋白(HbA1c)、空腹血糖(FPG)、餐后2h血糖(2hPG)和超氧化物歧化酶(SOD)水平,并检测角膜神经纤维密度(CNFD)、角膜神经分支密度(CNBD)和角膜神经纤维长度(CNFL),根据角膜神经损伤参数将患者分为损伤组与未损伤组。比较血液指标与角膜神经参数并通过Pearson相关性分析两者相关性。结果研究组HbA1c、FPG和2hPG水平高于对照组,SOD水平低于对照组(P<0.05)。研究组CNFD、CNBD和CNFL水平低于对照组(P<0.05)。研究组角膜神经损伤患者32例,未损伤患者48例。损伤组患者HbA1c、FPG和2hPG均高于未损伤组,SOD水平低于未损伤组(P<0.05)。损伤组患者CNFD、CNBD和CNFL均低于未损伤组(P<0.05)。相关性分析结果显示,CNFD、CNBD和CNFL与HbA1c、FPG和2hPG呈负相关,与SOD呈正相关(P<0.05)。结论T2DM患者的角膜神经损伤参数CNFD、CNBD和CNFL与血液指标HbA1c、FPG和2hPG呈正相关性,与血清SOD表达呈负相关性,患者角膜神经损伤越严重,机体血糖水平越高,血清SOD水平越低。Objective To analyze the expression of glycosylated hemoglobin(HbA1c)and superoxide dismutase(SOD)levels in type 2 diabetes mellitus(T2DM),and to discuss the correlation with corneal nerve injury.Methods Eighty patients with T2DM in Second People’s Hospital of Jiaozuo from December 2019 to December 2021 were enrolled as study group,and another 95 healthy individuals underwent physical examination during the same period were set as control group.HbA1c,fasting plasma glucose(FPG),2-hour postprandial blood glucose(2 h PG),SOD,corneal nerve structure parameters corneal nerve fiber density(CNFD),branch density(CNBD)and fiber length(CNFL)were detected.According to the corneal nerve structure parameters,the patients were divided into injury group and non-injury group.Blood parameters and corneal nerve parameters were compared between two groups,thereafter,Pearson correlation analysis was performed to discuss the correlation of those parameters.Results The levels of HbA1c,FPG and 2 h PG in the study group were higher than those in the control group,and the SOD level was lower than that in the control group(P<0.05).The levels of CNFD,CNBD and CNFL in the study group were lower than those in the control group(P<0.05).In study group,there were 32 patients with corneal nerve injury and 48 patients without corneal nerve injury.The HbA1c,FPG and 2 h PG of the injury group were higher than those of the non-injury group,and the SOD level was lower than that of the non-injury group(P<0.05).The CNFD,CNBD and CNFL of the injury group were lower than those of the non-injury group(P<0.05).The results of correlation analysis showed that CNFD,CNBD and CNFL were negatively correlated with HbA1c,FPG and 2 h PG,and positively correlated with SOD(P<0.05).Conclusion Corneal nerve structure parameters CNFD,CNBD and CNFL in T2DM patients are positively correlated with blood indexes HbA1c,FPG and 2 h PG,and negatively correlated with serum SOD activity,suggesting that patients with severe corneal nerve injury suffer a high blood glucose leve
