摘 要:目的探讨高频震荡通气联合气管内滴入布地奈德治疗新生儿重症肺炎的临床效果。方法选取2020年5月至2021年5月新乡市中心医院新生儿科收治的85例新生儿重症肺炎患儿作为研究对象,按随机数表法分成对照组(高频震荡通气治疗)和观察组(高频震荡通气联合气管内滴入布地奈德治疗)。比较两组患儿治疗前后动脉血气指标、临床症状消失时间、机械通气时间、住院时间、肺功能情况及并发症发生率。结果治疗后观察组患儿pH、动脉血氧分压(PaO_(2))改善更明显,差异有统计学意义(P<0.05),治疗后两组患儿动脉血二氧化碳分压(PaCO_(2))均下降,观察组患儿PaCO_(2)下降更明显(P<0.05)。观察组治疗后肺功能情况优于对照组(P<0.05)。观察组患儿口唇发绀、肺部啰音消失时间均短于对照组(P<0.05)。观察组机械通气时间、住院时间短于对照组(P<0.05);并发症发生率低于对照组(P<0.05)。结论气管内滴入布地奈德联合高频震荡通气治疗新生儿重症肺炎,可短时间缓解临床症状,改善动脉血气指标和肺功能,缩短机械通气时间及住院时间,效果明确,值得在临床上推广应用。Objective To investigate the clinical efficacy of high frequency oscillatory ventilation combined with intratracheal budesonide in the treatment of neonatal severe pneumonia.Methods From May 2020 to May 2021,85 childrens with severe neonatal severe pneumonia were enrolled in this study.They were divided into control group(high freguency oscillatory ventilation)and observation group(high freguency oscillatory ventilation combined with intratracheal budesonide treatment)according to the random number table method.The indexes of arterial blood gas indexes,the disappearance time of clinical symptoms,the duration of mechanical ventilation,the length of hospital stay,pulmonary function and the incidence of complications were compared between the two groups before and after treatment.Results After treatment,the improvement of pH and arterial partial pressure of oxygen(PaO_(2))in the observation group was more obvious(P<0.05).After treatment,arterial partial pressure of carbon dioxide(PaCO_(2))in the two groups was decreased,and the decrease of PaCO_(2) in the observation group was more significant(P<0.05).The pulmonary function of the observation group was better than that of the control group(P<0.05).The absence time of lip cyanosis and lung rales in observation group was shorter than that in control group(P<0.05).The mechanical ventilation time and hospital stay in the observation group were shorter than those in the control group(P<0.05).The incidence of complications was lower than that of control group(P<0.05).Conclusion Tracheal dripping of budesonide combined with high frequency oscillation ventilation in the treatment of neonatal severe pneumonia can alleviate clinical symptoms in a short time,improve arterial blood gas indexes and lung function,shorten the duration of mechanical ventilation and hospital stay,with clear clinical efficacy and worthy of clinical promotion and application.
