摘 要:目的探讨CT冠脉造影联合心肌血流灌注显像(MPI)技术对冠脉狭窄患者心肌缺血的评估价值。方法选取2019年4月至2021年4月商丘市中心医院收治的100例冠脉狭窄患者为研究对象,所有患者入院后均行CT冠脉造影联合MPI检查,并于1周内完成冠状动脉血管造影检查。以冠脉造影检查结果为金标准,分析CT冠脉造影联合MPI技术评估冠脉狭窄患者心肌缺血的价值。结果冠脉造影结果显示,100例冠脉狭窄患者中心肌缺血53例,占比为53%(53/100),无心肌缺血共47例,占比为47%(47/100)。经Kappa一致性检验,CT冠脉造影检查冠脉狭窄患者心肌缺血的结果和冠脉造影检查结果的一致性一般(Kappa=0.662,P<0.001);MPI检查冠脉狭窄患者心肌缺血的结果和冠脉造影检查结果的一致性一般(Kappa=0.741,P<0.001);CT冠脉造影联合MPI检查冠脉狭窄患者心肌缺血的结果和冠脉造影检查结果的一致性较好(Kappa=0.755,P<0.001)。以冠脉造影检查结果为金标准,CT冠脉造影联合MPI检查冠脉狭窄患者心肌缺血的准确度、灵敏度均高于CT冠脉造影或MPI单项检查,差异有统计学意义(P<0.05)。结论CT冠脉造影联合MPI技术在冠脉狭窄患者心肌缺血的评估中具有较高的应用价值,可提高诊断的准确度及灵敏度。Objective To investigate the value of CT coronary angiography combined with myocardial perfusion imaging(MPI)in the evaluation of myocardial ischemia in patients with coronary stenosis.Methods A total of 100 patients with coronary stenosis treated in Shangqiu Central Hospital from April 2019 to April 2021 were selected as the research subjects.All patients underwent CT coronary angiography combined with MPI after admission,and coronary angiography was completed within 1 week.Taking the results of coronary angiography as the gold standard,the value of CT coronary angiography combined with MPI in evaluating myocardial ischemia in patients with coronary stenosis was analyzed.Results The results of coronary angiography showed that among 100 patients with coronary stenosis,there were 53 cases of myocardial ischemia,accounting for 53%(53/100),and 47 cases of no myocardial ischemia,accounting for 47%(47/100).By Kappa consistency measure,the results of myocardial ischemia in patients with coronary stenosis by CT coronary angiography were generally consistent with those by coronary angiography(Kappa=0.662,P<0.001).The results of myocardial ischemia in patients with coronary stenosis by MPI were generally consistent with those of coronary angiography(Kappa=0.741,P<0.001).The results of myocardial ischemia in patients with coronary stenosis examined by CT coronary angiography combined with MPI were consistent with those of coronary angiography(Kappa=0.755,P<0.001).Taking the results of coronary angiography as the gold standard,the accuracy and sensitivity of CT coronary angiography combined with MPI for myocardial ischemia in patients with coronary stenosis were higher than those of CT coronary angiography or MPI(P<0.05).Conclusion CT coronary angiography combined with MPI has high application value in the evaluation of myocardial ischemia in patients with coronary stenosis,and can improve the accuracy and sensitivity of diagnosis.
