摘 要:高血压是导致心脑血管疾病的重要危险因素,在高血压的基础上更易发生心、脑、肾等器官的功能性减退及器质性损伤。因此,高血压的预防及早期治疗对于心脑血管病的防控尤为重要。运动性高血压是指在心肺运动测试过程中或刚结束时血压值超过正常反应性增高生理范围的现象,近年的研究表明,运动性高血压有助于发现临床标准测量条件下(静息时)未检出的有高血压或高血压伴器官损害发病风险的患者。运动时血压过高的人往往会在将来出现高血压,运动中收缩压的过度升高会增加左心室肥大、心肌梗死、脑血管卒中等心脑血管死亡的风险。本文讨论了运动性高血压的临床重要性及其治疗方案的最新进展,提出未来研究重点可能是确定适合我国人群的运动血压阈值,探索参与运动性高血压的病因和潜在生理学因素以及治疗效果。Hypertension is an important risk factor leading to cardiovascular and cerebrovascular diseases.On the basis of hypertension,functional decline and organic damage of organs such as heart,brain and kidney are more likely to occur.Therefore,the prevention and early treatment of hypertension are particularly important for the prevention and control of cardiovascular and cerebrovascular diseases.Exercise hypertension refers to the phenomenon that blood pressure value exceeds the physiological range of normal reactive increase in the process of cardiopulmonary exercise test or just ended.Recent studies have shown that exercise hypertension is helpful to discover patients at risk of developing hypertension or hypertension with organ damage that have not been detected under clinical standard measurement conditions(at rest).People with high blood pressure during exercise tend to develop hypertension in the future,while the widespread increase in systolic blood pressure during exercise increases the risk of cardiovascular death such as left ventricular hypertrophy,myocardial infarction and cerebrovascular stroke.This review discusses the clinical importance of exercise hypertension and the latest progress in its treatment,and suggest that future research may focus on determining exercise blood pressure threshold suitable for Chinese population,and to explore the etiology,potential physiological factors and treatment outcomes of exercise hypertension.
