外周静脉留置针失效的风险因素研究

摘　　要：目的:描述外周静脉留置针失效的现状及分析其影响因素,为预防留置针失效提供科学依据。方法:选取2017年5月~2018年2月在福州市某三级甲等综合性医院外科病房具有外周静脉留置针的556例患者,使用自制的留置针失效风险因素问卷测量留置针的失效情况,通过Kaplan—Meier单因素分析和Cox多因素分析探讨留置针失效的影响因素。结果:留置针失效率37.4%,其中堵管率12.2%,静脉炎发生率17.3%,移位发生率7.9%。超重、穿刺困难、穿刺疼痛、青霉素类抗生素,敷贴更换为堵管的危险因素。肘窝,敷贴更换为静脉炎的危险因素。二次穿刺,穿刺疼痛,敷贴更换为移位的危险因素。结论:在临床实践中应加强关注超重、穿刺疼痛,穿刺困难,输注青霉素类药物患者,提高穿刺技术,避免二次穿刺,避免肘窝及手腕的穿刺部位,慎重留置针型号选择。Objective:To describe the current situation of the failure of peripheral venous indwelling needle and analyze its influencing factors,so as to provide scientific basis for preventing the failure of indwelling needle.Methods:From May 2017 to February 2018,556 patients with peripheral venous indwelling needles in the surgical ward of a tertiary grade A general hospital in Fuzhou City were selected,and the self-made indwelling needle failure risk factor questionnaire was used to measure the failure of indwelling needles,through Kaplan-Meier univariate analysis and Cox multivariate analysis to explore the influencing factors of indwelling needle failure.Results:The failure rate of indwelling needle was 37.4%,including 12.2%of tube blockage,17.3%of phlebitis and 7.9%of displacement.Overweight,puncture difficulty,puncture pain,penicillin antibiotics,replacement of application with tube plugging were the risk factors.The risk factors of phlebitis in cubital fossa and application replacement.The risk factors of secondary puncture,puncture pain,application replacement for displacement.Conclusion:In clinical practice,we should pay more attention to overweight,puncture pain,puncture difficulty,infusion of penicillins,improve puncture technology,avoid secondary puncture,avoid elbow fossa and wrist puncture site,and carefully choose the type of indwelling needle.
