胎心监护仪对降低剖宫产率的影响研究

摘　　要：目的:探讨实施胎心监测仪对降低剖宫产率及改善妊娠结局的效果。方法:选取2019年8月~2020年8月行产前检查的产妇102例,所有纳入对象均接受胎心监护,结合产妇胎心率曲线、宫缩曲线评定产妇情况,将胎心率曲线正常者(记作对照组)纳入66例,将胎心率曲线异常者(记作研究组)纳入36例,根据产妇情况实施处理措施,对分娩方式、妊娠合并症及并发症情况、新生儿结局等指标分析对比。结果:经统计,研究组阴道分娩率相对低、剖宫产率更高,且研究组产妇脐带异常、羊水过多或过少占比相较对照组更高,新生儿发生缺氧缺血性脑病、羊水污染率相较对照组更高,均具有统计学差异(P<0.05)。结论:临床产科医师可根据胎心监护对产妇宫内情况详细掌握,准确判定胎儿有无宫内窘迫,以便于及时发现异常情况,选取合理的分娩方式及对新生儿窒息予以处理,降低剖宫产率及各类新生儿疾病。Objective:To explore the effect of using fetal heart monitor to reduce the rate of cesarean section and improve the pregnancy outcome.Methods:A total of 102 parturients who underwent prenatal examination from August 2019 to August 2020 were selected.All the subjects were monitored by fetal heart rate monitor,the patients with normal fetal heart rate curve(as control group)were included in 66 cases,and the patients with abnormal fetal heart rate curve(as study group)were included in 36 cases,methods of delivery,pregnancy complications and complications,neonatal outcomes and other indicators analysis and comparison.Results:Compared with the control group,the rate of vaginal delivery was lower,the rate of cesarean section was higher,and the rate of abnormal umbilical cord,oligohydramnios or oligohydramnios was higher in the study group than in the control group,the incidence of hypoxic-ischemic encephalopathy(Hie)and amniotic fluid contamination in neonate was higher than that in control group(P<0.05).Conclusion:The clinical obstetricians can accurately determine whether the fetus has intrauterine distress according to the fetal heart monitoring,so as to find out the abnormal situation in time,select the reasonable delivery mode and deal with the asphyxia of the newborn,to reduce the rate of cesarean section and all kinds of neonatal diseases.
