摘  要:起搏器植入是现代维持心率、提高生存率、改善患者生活质量及疾病远期预后的重要手段,故判断其电极的稳定性尤为重要。除传统的评价导线稳定性的方法,损伤电流(current of injury,COI)在判断起搏器电极稳定性方面同样有极其重要的作用。COI是指在起搏电极植入的过程中发现电极与心内膜接触后会在短期内产生一定的电位变化,类似于心肌缺血损伤,主要通过测量ST段抬高的幅度和宽度判断其大小。COI可预测急性期及中期电极稳定性,减少电极的脱位、穿孔几率,预测随访期间电极的稳定性,是起搏器正常工作的保障。对于希蒲系统起搏[希氏束起搏(His-Bundle Pacing,HBP)及左束支起搏(Left bundle branch pacing,LBBP)]的电极稳定性也有着重要的指导作用。Pacemaker implantation is an important means to maintain the heart rate,to improve the survival rate and the quality of life of patients,and the long-term prognosis of the disease.Therefore,it is particularly important to judge the stability of its electrode.In addition to the traditional method of evaluating the stability of the conductor,the current of injury(COI)is also very important in judging the stability of the pacemaker electrode.In the process of the so-called COI pacing electrode implantation,it was found that certain potential changes would occur in the short term after the electrode contacted the endocardium,which was similar to myocardial ischemia injury,and the magnitude of ST segment elevation was determined mainly by measuring the amplitude and width of ST segment elevation.COI can predict the stability of the electrode in the acute phase and the middle phase,reduce the probability of dislocation and perforation of the electrode,and predict the stability of the electrode during the follow-up,which is the guarantee of the normal work of the pacemaker.It also plays an important role in guiding the electrode stability of his-bundle pacing(HBP) and left bundle branch pacing(LBBP).
