闭合复位内固定治疗闭合性胫腓骨骨折的有效性

摘　　要：目的观察闭合复位内固定治疗闭合性胫腓骨骨折的有效性。方法选取2017年3月至2020年5月收治的84例闭合性胫腓骨骨折患者,根据随机数字表法将其分为治疗组(闭合复位内固定)及对照组(切开复位内固定),各42例。比较两组的治疗效果。结果术后1 d,治疗组的血清IL-6、CRP、TNF-α水平显著低于对照组(P<0.05)。术后1个月,治疗组的血清TGF-β_(1)、IGF-1水平显著高于对照组(P<0.05)。治疗组的手术时间、住院总时长、骨痂形成时间、骨折愈合时间均显著短于对照组(P<0.05)。术后3、6个月,两组患者的踝关节功能评分均明显高于术前(P<0.05);术后3个月,治疗组的踝关节功能评分显著高于对照组(P<0.05);术后6个月,两组的踝关节功能评分比较,差异无统计学意义(P>0.05)。结论相比切开复位内固定,闭合复位内固定治疗闭合性胫腓骨骨折具有创伤性小、患者术后康复快、炎症反应轻等优点,整体应用效果显著。Objective To observe the effectiveness of closed reduction and internal fixation in the treatment of closed tibia and fibula fractures.Methods A total of 84 patients with closed tibia and fibula fractures admitted in our hospital from March 2017 to May 2020 were selected and divided into treatment group(closed reduction and internal fixation)and control group(open reduction and internal fixation)according to random number table method,with 42 cases in each group.The therapeutic effects of the two groups were compared.Results One day after operation,the levels of serum IL-6,CRP and TNF-αin the treatment group were significantly lower than those in the control group(P<0.05).One month after operation,the levels of serum TGF-β_(1) and IGF-1 in the treatment group were significantly higher than those in the control group(P<0.05).The operation time,total hospital stay,callus formation time and fracture healing time in the treatment group were significantly shorter than those in the control group(P<0.05).At 3 and 6 months after operation,the ankle function score of the two groups was significantly higher than that before operation(P<0.05);three months after operation,the ankle function score of the treatment group was significantly higher than that of the control group(P<0.05);six months after operation,there was no significant difference in ankle function score between the two groups(P>0.05).Conclusion Compared with open reduction and internal fixation,closed reduction and internal fixation in the treatment of closed tibia and fibula fractures has the advantages of less trauma,rapid postoperative recovery and light inflammatory reaction.The overall application effect is remarkable.
