摘  要:目的:构建采用局部枸橼酸抗凝方法进行连续性血液净化治疗的离子钙监测优化方案。方法:以《重症血液净化》为基础,通过检索国内外文献、病例回顾拟订方案初稿,采用德尔菲法进行专家函询,确定最终方案。结果:共进行了2轮专家函询,问卷有效回收率均为100%,专家权威系数分别为0.943,0.964,肯德尔和谐系数分别为0.172,0.186(P<0.01)。最终确立的局部枸橼酸抗凝连续性血液净化治疗离子钙监测优化方案包括一级指标3项、二级指标11项。结论:构建的局部枸橼酸抗凝连续性血液净化治疗离子钙监测优化方案具有较高的科学性、可靠性,可为采用局部枸橼酸抗凝的连续性血液净化治疗提供临床实践指导。Objective:To construct optimal monitoring scheme of ionic calcium in local citrate anticoagulation continuous blood purification treatment. Methods:Based on Severe Blood Purification,the first draft of the scheme was drawn up by searching the literature,case review,and the final scheme was determined by Delphi method. Results:Two rounds of expert correspondence were conducted. The effective recovery rates of 2 rounds of expert letter questionnaires were 100%,and expert authority coefficients were 0. 943 and 0. 964respectively. The Kendall coordination coefficients of 2 rounds were 0. 172 and 0. 186 respectively(P<0. 01). The formed optimal monitoring scheme of ionic calcium in local citrate anticoagulation continuous blood purification treatment included 3 first-level indexes and 11 second-level indexes. Conclusions:The optimal monitoring scheme of ionic calcium in local citrate anticoagulation continuous blood purification treatment has high scientificity,reliability and safety,which can provide clinical practice guidance for local citrate anticoagulation continuous blood purification treatment.
