摘 要:目的评价计步器干预对慢性阻塞性肺疾病(COPD)患者体力活动水平、运动能力以及生活质量的影响。方法计算机检索中国知网、万方数据库、维普数据库、中国生物医学文献数据库、PubMed、Cochrane Library、Web of Science、Embase发表的关于计步器干预COPD人群体力活动、运动能力、生活质量的临床随机对照试验,检索时限为建库至2021年8月6日。使用RevMan 5.3软件进行数据合成与分析。结果最终纳入20篇文献,其中英文文献15篇,中文文献5篇,样本量总计2563例。经亚组分析发现,计步器干预可以提高年龄>65岁、采用不同方式进行反馈、干预时长<3个月或3~12个月的COPD患者的体力活动水平(每日步数)(P<0.05)。以干预组是否有理论指导为区分进行亚组分析后发现,计步器干预可以提高无理论指导组6 min步行试验距离(P<0.01)。Meta分析结果显示,计步器干预对COPD患者生活质量、FEV1/FVC无影响(P>0.05),对FEV1%预计值有影响(P=0.004)。结论计步器干预可以增加COPD患者的体力活动水平及运动能力,改善患者肺功能,但对生活质量无明显改善作用。Objective To evaluate effect of pedometer intervention on physical activity level,exercise ability and quality of life in patients with chronic obstructive pulmonary disease(COPD).Methods The clinical randomized controlled trials on pedometer intervention in physical activity,exercise ability and quality of life of COPD population published by CNKI,Wanfang database,VIP database,China Biology Medicine disc,PubMed,Cochrane Library,Web of Science and Embase were searched by computer.The retrieval time limit was from the establishment of the databases to August 6,2021.Revman 5.3 software was used for data synthesis and analysis.Results Finally,a total of 20 literatures were included,including 15 English literatures and 5 Chinese literatures,with a total sample size of 2563 cases.In subgroup analysis,pedometer intervention increased physical activity(daily steps)in COPD patients older than 65 years,who received feedback in different ways and whose intervention duration was less than 3 months or 3 to 12 months(P<0.05).According to whether the intervention group had theoretical guidance or not,the subgroup analysis showed that pedometer intervention could improve the 6-min walking test distance of the group without theoretical guidance(P<0.01).Meta-analysis showed that pedometer intervention had no effect on quality of life and FEV1/FVC of COPD patients(P>0.05),but had an effect on FEV1%predicted value(P=0.004).Conclusions Pedometer intervention can increase physical activity level and exercise ability of COPD patients and improve lung function,but it has no significant improvement in quality of life.
