摘　　要：目的探讨血栓弹力图(TEG)评价低相对分子质量肝素(LMWH)对重度子痫前期患者凝血功能的改善效果。方法以76例重度子痫前期患者为对象,随机分为观察组(LMWH)与对照组(常规治疗)。对比两组凝血指标及TEG参数。结果治疗后观察组纤维蛋白原(Fbg)水平低于对照组(P<0.05);活化部分凝血活酶时间(aPTT)、凝血酶原时间(PT)凝血酶时间(TT)水平均高于对照组(P>0.05)。治疗后观察组D二聚体(D-D)水平低于对照组(P<0.05);两组血小板计数(PLT)治疗前后未见差异(P>0.05)。治疗后观察MA值、Angle角低于对照组,R值、K值高于对照组(P<0.05)。结论TEG监测可反映LMWH对重度子痫前期患者凝血功能的改善情况,可作为指导子痫前期早期防治的重要手段。Objective To evaluate the effect of low molecular weight heparin(LMWH)on coagulation function in patients with severe preeclampsia by thromboelastography(TEG).Methods 76 patients with severe preeclampsia were randomly divided into observation group(low molecular weight heparin)and control group(routine treatment).The coagulation indexes and TEG parameters of the two groups were compared.Results After treatment,the level of fibrinogen(Fbg)in the observation group was lower than that in the control group(P<0.05);There in activated partial thromboplastin time(aPTT),prothrombin time(PT)and thrombin time(TT)higher than control group(P<0.05)After treatment,the level of D-dimer(D-D)in the observation group was lower than that in the control group(P<0.05);There was no difference in platelet count(PLT)between the two groups before and after treatment(P>0.05).After treatment,the Ma value and angle angle were lower than those in the control group,and the R value and K value were higher than those in the control group(P<0.05).Conclusion TEG monitoring can reflect the improvement of coagulation function of LMWH in patients with severe preeclampsia,and can be used as an important means to guide the early prevention and treatment of preeclampsia.
