摘　　要：目的了解Stanford A型主动脉夹层患者术后谵妄的症状感受和应对体验。方法采用目的抽样法,于2020年7月—2021年7月选取河南省胸科医院心血管外科收治的13例Stanford A型主动脉夹层术后谵妄患者为研究对象,采用现象学研究法对患者进行半结构式访谈,采用Colaizzi 7步分析法对访谈资料进行分析。结果Stanford A型主动脉夹层患者的术后谵妄感受可归纳为意识错乱、身心不适、认知紊乱、死亡感知4个次主题,应对体验可归纳为沟通交流障碍、鼓励支持缺失、安全陪伴需求3个次主题。结论Stanford A型主动脉夹层术后谵妄患者存在诸多不适症状,渴望得到支持和帮助。Objective To understand the symptom perception and coping experience of postoperative delirium in patients with Stanford type A aortic dissection(TAAD).Methods Using the purposive sampling method,a total of 13 patients with delirium after Stanford type A aortic dissection who were admitted to Department of Cardiovascular Surgery of Henan Provincial Chest Hospital from July 2020 to July 2021 were selected as the research objects.Semi-structured interviews were conducted with patients using the phenomenological research method and the interview data were analyzed using the Colaizzi 7-step analysis method.Results Postoperative delirium in patients with TAAD was classified into four sub-themes,such as confusion,physical and mental discomfort,cognitive disturbance and death perception.The coping experience was classified into three sub-themes,such as communication barriers,lack of encouragement and support and need for safe companionship.Conclusions Patient with delirium after TAAD have many uncomfortable symptoms and long for support and help.
