摘 要:目的:利用16S rRNA基因测序技术观察加味人参养荣汤对气虚证临床特征及肠道微生态的影响。方法:给予20例气虚证受试者加味人参养荣汤治疗8周。采用气虚证症状分级量化表(QiS)、疲劳量表-14(FS-14)、疲劳自评量表(FSAS)和多维疲劳量表-20(MFI-20)评估治疗后患者的临床特征变化。通过16S rRNA基因测序观察患者治疗前后粪便肠道微生物丰度、分布及组成结构的变化。采用相关性分析探讨临床特征与肠道微生物之间的关联性,并利用Gephi将其可视化。结果:加味人参养荣汤可以显著降低气虚证患者QiS、FS-14、FSAS和MFI-20量表评分(P<0.01);alpha多样性分析表明,干预前后患者肠道微生物的observed species指数、chao指数、ace指数、shannon指数、simpson指数和coverage指数未出现显著变化;beta多样性分析表明,干预前后患者肠道微生物组成结构在属分类水平出现显著区分;治疗后,p_Proteobacteria的相对丰度显著上调(P<0.05);相关性分析表明,Bacteroides、Clostridium和Phascolarctobacterium可能与其气虚证病情加重有关,Bifidobacterium、Lactobacillus、Enterococcus、Escherichia、Dialister、Gemmiger、Mitsuokella、Paraprevotella和Eubacterium可能与其气虚证病情缓解有关。结论:加味人参养荣汤可以显著改善气虚证患者的临床症状,其作用机制可能与调节肠道微生物丰度、分布及组成结构有关。Objective:To observe the effects by of Modified Renshen Yangrong Decoction on clinical characteristics and gut microbiota of patients with qi-deficiency syndrome by 16S rRNA gene sequencing.Methods:Twenty subjects with qideficiency syndrome were treated with Modified Renshen Yangrong Decoction for 8 weeks.QiS,FS-14,FSAS,and MFI-20 were used to evaluate the change of clinical characteristics after treatment.The changes of gut microbiota in the abundance,distribution,and composition were observed before and after treatment by 16S rRNA gene sequencing.Correlation analysis was used to compare the association between clinical characteristics and gut microbiota,and Gephi was used to visualize it.Results:Modified Renshen Yangrong Decoction could significantly reduce the scores of QiS,FS-14,FSAS,and MFI-20 in patients with qi-deficiency syndrome(P<0.01);Alpha diversity analysis showed that the observed species index,chao index,ace index,shannon index,simpson index,and coverage index did not change significantly before and after intervention.Beta diversity analysis showed that the composition of gut microbiota before and after the intervention was significantly changed at the level of genus classification.After treatment,the relative abundance of p_Proteobacteria was significantly increased(P<0.05);Correlation analysis showed that Bacteroides,Clostridium,and Phascolarctobacterium may be related to the aggravation of qi-deficiency syndrome.Bifidobacterium,Lactobacillus,Enterococcus,Escherichia,Dialister,Gemmiger,Mitsuokella,Paraprevotella,and Eubacterium may be related to the mitigation of qi-deficiency syndrome.Conclusion:Modified Renshen Yangrong Decoction has a good effect on relieving the clinical symptoms of patients with qi-deficiency syndrome,and the mechanism may be related to the regulation of abundance,distribution,and composition in gut microbiota.
