益艾康胶囊对HIV/AIDS免疫重建不良肺脾气虚证患者肠道菌群与免疫功能的影响

摘　　要：目的:探讨益艾康胶囊对HIV/AIDS免疫重建不良肺脾气虚证患者肠道菌群与免疫功能的影响。方法:收集2019年9月至2020年5月艾滋病高效抗反转录病毒治疗(HA A RT)后免疫重建不良肺脾气虚证患者18例,给予益艾康胶囊(5粒/次,3次/d)口服24周,于治疗前后分别采集1次外周血和粪便标本,用流式细胞技术和16S rDNA方法检测各样本T淋巴细胞亚群,CD4/CD8细胞比例,Th17、Treg细胞表达率及Th17/Treg细胞比例和肠道菌群情况。结果:与治疗前比较,治疗后Tr e g细胞表达率显著降低(P<0.01),C D4+T细胞计数、CD8+T细胞计数、Th17细胞表达率及Th17/Treg细胞比例均显著升高(P<0.01,P<0.05)。与治疗前比较,治疗后患者肠道菌群中Lachnoclostridium菌、鼠杆菌、乳酸杆菌、α变形杆菌纲、气单胞菌目以及普雷沃氏菌属丰度有所下降(P<0.05),梭杆菌和毛螺旋菌丰度显著升高(P<0.05)。结论:益艾康胶囊可以提高HI V/A I D S免疫重建不良肺脾气虚证患者的CD4+T细胞计数,促使Th17/Tr eg细胞比例趋于平衡,有效调节肠道菌群紊乱,增强机体免疫。Objective:To explore the effects of Yiaikang Capsules on intestinal microflora and immune function in HIV/AIDS patients with poor immune reconstitution in lung and spleen qi deficiency syndrome.Methods:A total of 18 HIV/AIDS patients with lung and spleen qi deficiency syndrome who had poor immune reconstitution after highly active antiretroviral therapy(HAART)were collected from September 2019 to May 2020.They were given Yiaikang Capsules(5 capsules per time,3 times per day)for 24 weeks orally,and peripheral blood and stool samples were collected once before and after treatment.Flow cytometry and 16S rDNA method were used to detect the T lymphocyte subsets,the proportion of CD4/CD8 cells,Th17 and Treg cells expression,and Th17/Treg cells and intestinal microflora in each sample.Results:Compared with before the treatment,the expression rate of Treg cells after the treatment was significantly decreased(P<0.01),and CD4+T cell count,CD8+T cell count,Th17 cell expression rate and Th17/Treg cell ratio were significantly increased(P<0.01,P<0.05).Compared with before treatment,the abundance of Lachnoclostridium,Muribaculaceae,Lactobacillaceae,Alphaproteobacteria,Aeromonadales and Prevotella in the intestinal flora of patients decreased after treatment(P<0.05),while the abundance of Fusobacteriota and Lachnospiraceae was significantly increased(P<0.05).Conclusion:Yiaikang Capsule can improve the CD4+T cell count in HIV/AIDS patients with poor immune reconstitution after HAART in lung and spleen qi deficiency syndrome,promote the balance of Th17/Treg cell ratio,effectively regulate intestinal flora disorder and enhance body immunity.
