摘  要:目的:探究银莱汤对胃肠积热合并肺炎大鼠Th17/Treg细胞平衡的影响,探讨银莱汤治疗胃肠积热合并肺炎的机制。方法:将30只SPF级大鼠随机分为正常组、模型组、银莱汤治疗组,每组10只。模型组与银莱汤治疗组采用高热量饮食联合LPS雾化的方式制备胃肠积热合并肺炎模型,银莱汤治疗组给予银莱汤(0.5 g·mL-1·100 g-1)灌胃治疗。每日监测大鼠进食量、体质量、肛温等指标;采用HE染色观察肺组织炎性损伤情况;ELISA法检测各组大鼠血清中TNF-α、IL-1β、IL-6、IL-17A、TGF-β、IL-10等细胞因子水平;流式细胞术分析外周血及肠系膜淋巴结(MLNs)中Th17、Treg细胞比例;RT-PCR检测肺组织RORγt、Foxp3 mRNA表达。结果:与模型组比较,银莱汤治疗组肺部炎性损伤明显缓解,血清TNF-α、IL-1β、IL-6水平显著降低(P<0.05,P<0.01);外周血及MLNs中Th17细胞比例显著下降,Treg细胞比例显著升高(P<0.01,P<0.05);同时,银莱汤治疗组外周血IL-17A表达显著降低(P<0.05),TGF-β、IL-10表达显著升高(P<0.05);肺组织RORγt mRNA表达显著降低(P<0.01),Foxp3 mRNA表达显著升高(P<0.01)。结论:银莱汤可有效缓解胃肠积热合并肺炎大鼠肺组织炎性损伤,其作用机制可能与调节Th17/Treg细胞平衡有关。Objective:To explore the effects of Yinlai Decoction on the balance of Th17/Treg cells in rats with gastrointestinal heat retention syndrome and pneumonia,and to explore the mechanism of Yinlai Decoction in the treatment of gastrointestinal heat retention syndrome and pneumonia.Methods:Thirty SPF rats were randomly divided into normal group,model group and treatment group,10 rats in each group.The model group and the treatment group were treated with high-calorie diet combined with LPS atomization to prepare a model of gastrointestinal heat accumulation with pneumonia,and the treatment group was given Yinlai Decoction(0.5 g·mL-1·100 g-1)by gavage.The food-intake,body masss and rectal temperature were monitored daily.HE staining was used to observe the inflammatory damage of lung tissue;ELISA method was used to detect serum levels of TNF-α,IL-1β,IL-6,IL-17A,TGF-β,IL-10 and other cytokines in each group of rats;The proportion of Th17 and Treg cells in peripheral blood and mesenteric lymph nodes was analyzed by cytometry;The expression of RORγt and Foxp3 mRNA in lung tissue was detected by RT-PCR.Results:Compared with the model group,the inflammatory injury in the lungs of rats in the treatment group was significantly relieved,and the levels of serum TNF-α,IL-1β,IL-6 were significantly reduced(P<0.05,P<0.01);The proportion of Th17 cells in blood and MLNs decreased,and the proportion of Treg cells increased(P<0.01,P<0.05);at the same time,the expression of IL-17A in peripheral blood of rats in the treatment group decreased(P<0.05),and the expression of TGF-βand IL-10 increased(P<0.05);the expression of RORγt mRNA in lung tissue decreased(P<0.01),and the expression of Foxp3 mRNA increased(P<0.01).Conclusion:Yinlai Decoction can effectively relieve the inflammatory injury of lung tissue in rats with gastrointestinal fever and pneumonia,and its mechanism of action may be related to the regulation of Th17/Treg cell balance.
