摘　　要：目的探讨不同消毒方案在预防维持性透析患者带隧道带涤纶套导管(TCC)相关感染性湿疹样皮炎(IED)中的效果。方法选取2020年4月~2020年10月于三六三医院接受每周三次维持性血液透析治疗的85例带隧道带涤纶套透析导管患者作为研究对象。将患者随机分为观察组(43名)和对照组(42名)。安尔碘组采用安尔碘消毒方案,聚维酮碘组采用聚维酮碘消毒方案。随访6个月,观察记录两组患者导管出皮口处皮肤情况,包括IED发生率及不良反应发生情况。结果观察组IED的发生率上低于对照组,差异有统计学意义(P<0.05);两组患者IED严重程度观察组低于对照组(P<0.05);两组不良反应发生率差异无统计学意义(P>0.05);两组患者皮疹处分泌物病源菌分布示观察组培养结果阳性的菌珠量少于对照组(P<0.05),且对万古霉素、利奈唑胺、利福平等普遍敏感。结论在血透患者带隧道带涤纶套导管相关感染性湿疹样皮炎的预防方面,安尔碘组方案优于聚维酮碘组方案。安尔碘消毒方案对预防TCC相关的出皮口处感染性湿疹样皮炎有良好帮助。Objective To explore the effect of preventive effect of different disinfection schemes on infectious eczema-like dermatitis in maintenance dialysis patients with tunneled cuffed catheter.Methods 85 maintenance dialysis patients with tunneled cuffed catheter were randomly divided into aneriodine group(n=43)and povidone iodine group(n=42).Two groups of patients were treated with aneriodine disinfection regimen and povidone iodine disinfection regimen respectively.The patients were followed up for 6 months,and the conditions of the skin at catheter outlet in the two groups were observed and recorded,including the incidence of IED and adverse reactions.Results The incidence of IED in aneriodine group was lower than that in povidone iodine group,the difference was statistically significant(P<0.05),but there was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Aneriodine group is superior to povidone iodine group in the prevention of infectious eczema-like dermatitis associated with tunneled cuffed catheter in hemodialysis patients.Anerdide disinfection schemes is helpful for the prevention of infectious eczema-like dermatitis in maintenance dialysis patients with tunneled cuffed catheter.
