经脐单孔腹腔镜下睾丸下降固定术治疗小儿腹股沟区隐睾的临床观察

摘　　要：目的探讨经脐单孔腹腔镜下睾丸下降固定术治疗小儿腹股沟区隐睾的临床疗效。方法抽取2018年5月至2020年6月聊城市第二人民医院收治的腹股沟区隐睾患儿84例,按随机数字表法分为对照组和观察组,每组42例。对照组采用传统经腹股沟切口手术,观察组采用经脐单孔腹腔镜下睾丸下降固定术。比较两组手术相关指标、术后24 h血清疼痛应激因子[神经肽Y(NPY)、神经生长因子(NGF)、P物质(SP)]水平、术后7 d性激素水平及并发症发生情况。结果观察组术中出血量[(5.75±0.49)ml]少于对照组[(9.52±1.22)ml],术后恢复正常活动时间、住院时间[(1.50±0.30)、(6.52±0.65)d]均短于对照组[(1.89±0.46)、(6.94±1.13)d],P均<0.05。术后24 h,观察组血清NPY、NGF、SP水平均低于对照组(P均<0.05)。术后7 d,两组血清睾酮、双氢睾酮、促卵泡激素水平比较差异未见统计学意义(P>0.05)。两组并发症发生率比较差异未见统计学意义(P>0.05)。结论经脐单孔腹腔镜下睾丸下降固定术治疗小儿腹股沟区隐睾效果显著,创伤小且术后恢复时间短,可有效缓解患儿疼痛,安全有效,有利于其恢复,且外观美观。Objective To investigate the clinical effect of single-port laparoscopic testicular descent fixation in the treatment of cryptorchidism in the inguinal region of children.Methods Eighty-four children with cryptorchidism in the inguinal region who were admitted to the Second People’s Hospital of Liaocheng from May 2018 to June 2020 were selected,and they were divided into control group and observation group by random number table method,with 42 cases in each group.Patients in control group were treated by conventional inguinal surgery,while patients in observation group were treated by single-port laparoscopic testicular descent fixation.The operation related indicators,serum pain stress factors levels,including neuropeptide Y(NPY),nerve growth factor(NGF),and substance P(SP)at 24 h after operation,sex hormone levels 7 days after operation,and complications were compared between the two groups.Results The amount of intraoperative bleeding in observation group was(5.75±0.49)ml,less than the(9.52±1.22)ml in control group(P<0.05);the postoperative recovery time and hospital stay of observation group were(1.50±0.30)d and(6.52±0.65)d,shorter than the(1.89±0.46)d and(6.94±1.13)d of control group(all P<0.05).The levels of serum NPY,NGF and SP in observation group were lower than those in control group at 24 h after operation(all P<0.05).There was no significant difference in the levels of serum testosterone,dihydrotestosterone and follicle stimulating hormone between the two groups 7 days after operation(P>0.05).There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).Conclusions Single-port laparoscopic testicular descent fixation is effective in the treatment of cryptorchidism in the inguinal region of children,with little trauma and short postoperative recovery time.It can effectively relieve pain,which is safe and effective.
