千金苇茎汤合小陷胸汤加减联合常规疗法治疗老年支气管扩张合并感染临床研究  

Clinical Study on Modified Qianjin Weijing Tang and Xiaoxianxiong Tang Combined with Routine Therapy for Bronchiectasis Complicated with Infection in Senile Patients

摘　　要：目的:观察千金苇茎汤合小陷胸汤加减联合常规疗法治疗老年支气管扩张合并感染的临床疗效。方法:选取老年支气管扩张合并感染患者84例,随机分为对照组和试验组各42例。对照组予西医常规治疗,试验组在对照组基础上联合千金苇茎汤合小陷胸汤加减治疗,均治疗2周。比较2组临床疗效及治疗前后外周血降钙素原(PCT)、C-反应蛋白(CRP)、CD4+CD25+调节性T淋巴细胞(Treg)、白细胞介素-2 (IL-2)、白细胞介素-10 (IL-10)水平。结果:治疗后,试验组临床总有效率95.2%,高于对照组81.0%(P<0.05)。治疗后,2组中医证候积分、PCT、CRP和IL-2水平较治疗前下降(P<0.05),且试验组上述指标均明显低于对照组(P<0.05);2组Treg、IL-10较治疗前升高(P<0.05),且试验组Treg、IL-10水平明显高于对照组(P<0.05)。结论:千金苇茎汤合小陷胸汤加减联合常规疗法治疗老年支气管扩张合并感染具有良好的协同作用,可显著改善患者症状,抑制体内炎症反应,增强免疫调节功能,促进免疫稳态平衡的恢复。Objective:To observe the clinical effect of modified Qianjin Weijing tang and Xiaoxianxiong tang combined with routine therapy for bronchiectasis complicated with infection in senile patients.Methods:A total of 84 cases of senile patients with bronchiectasis complicated with infection were selected and randomly divided into the control group and the experiment group,with 42 cases in each group.The control group was given routine western medicine treatment,and the experiment group was additionally treated with modified Qianjin Weijing tang and Xiaoxianxiong tang.Both groups were treated for two weeks.The levels of procalcitonin(PCT),C-reactive protein(CRP),CD4+CD25+regulatory T lymphocyte(Treg),interleukin-2(IL-2) and interleukin-10(IL-10) in peripheral blood before and after treatment as well as the clinical effects in the two groups were compared.Results:After treatment,the total clinical effective rate was 95.2% in the experiment group,higher than that of 81.0% in the control group(P<0.05).After treatment,Chinese medicine syndrome scores and the levels of PCT,CRP and IL-2 in the two groups were decreased when compared with those before treatment(P<0.05),and the above indexes in the experiment group were significantly lower than those in the control group(P<0.05);the levels of Treg and IL-10 in the two groups were increased when compared with those before treatment(P<0.05),and the levels of Treg and IL-10 in the experiment group were significantly higher than those in the control group(P<0.05).Conclusion:The therapy of modified Qianjin Weijing tang and Xiaoxianxiong tang combined with routine therapy for bronchiectasis complicated with infection in senile patients has a good synergistic effect,and can significantly improve their symptoms,inhibit inflammatory responses in vivo,enhance immunoregulatory function and promote the recovery of immune homeostasis balance.
