天麻活血汤联合盐酸氟桂利嗪胶囊治疗脑供血不足性眩晕症临床研究

摘　　要：目的:观察天麻活血汤联合盐酸氟桂利嗪胶囊治疗脑供血不足性眩晕症的临床疗效。方法:选取脑供血不足性眩晕症患者125例,按随机数字表法分成对照组62例和观察组63例。对照组给予盐酸氟桂利嗪胶囊口服,观察组在对照组基础加用天麻活血汤治疗。比较2组治疗前后眩晕症状积分、椎动脉型颈椎病功能评定量表(FS-CSA)评分,检测血清学指标[血小板活化因子(PAF)、血栓素B2 (TXB2)、白细胞介素-8 (IL-8)]和血液流变学指标,评价临床疗效和不良反应发生情况。结果:观察组总有效率为95.24%,高于对照组82.26%,差异有统计学意义(P<0.05)。治疗后,2组眩晕程度、眩晕发作次数、持续时间评分、FS-CSA评分均低于治疗前(P<0.05),且观察组治疗后眩晕程度、眩晕发作次数、持续时间评分、FS-CSA评分低于对照组(P<0.05)。治疗后,2组PAF、TXB2、IL-8水平低于治疗前(P<0.05),且观察组治疗后PAF、TXB2、IL-8水平均低于对照组(P<0.05)。治疗后,2组全血黏度、血浆黏度、红细胞比容、纤维蛋白原水平较治疗前降低,且观察组血液流变学指标均低于对照组(P<0.05)。结论:天麻活血汤联合盐酸氟桂利嗪胶囊治疗脑供血不足性眩晕症临床疗效较好,能改善患者眩晕症状,调节血清学指标和血液流变学指标,提高患者生活质量。Objective:To observe the clinical effect of Tianma Huoxue tang combined with flunarizine hydrochloride capsules for vertigo symptom due to insufficiency of cerebral blood supply.Methods:A total of 125 cases of patients with vertigo symptom due to insufficiency of cerebral blood supply were selected and divided into the control group and the observation group according to the random number table method,with 62 and 63 cases in each group respectively.The control group was given the oral administration with flunarizine hydrochloride capsules,and the observation group was additionally treated with Tianma Huoxue tang based on the treatment of the control group.The vertigo symptom scores and scores of functional scale for cervical spondylosis of vertebral artery type(FS-CSA) were compared between the two groups before and after treatment,serological indexes [platelet activating factor(PAF),thromboxane B2(TXB2),interleukin-8(IL-8)]and hemorheological indexes were detected in the two groups.Clinical effect and adverse reactions were evaluated.Results:The total effective rate was 95.24% in the observation group,higher than that of 82.26% in the control group,the difference being significant(P<0.05).After treatment,vertigo degree,vertigo frequency,and scores of duration and FS-CSA in the two groups were lower than those before treatment(P<0.05),and vertigo degree,vertigo frequency,and scores of duration and FS-CSA in the observation group were lower than those in the control group(P<0.05).After treatment,levels of PAF,TXB2 and IL-8 in the two groups were lower than those before treatment(P<0.05),and the three levels in the observation group were lower than those in the control group(P<0.05).After treatment,whole blood viscosity,plasma viscosity,hematocrit and fibrinogen levels in the two groups were decreased when compared with those before treatment,and the above four levels in the observation group were lower than those in the control group(P<0.05).Conclusion:Tianma Huoxue tang combined with flunarizine hydrochloride cap
