摘　　要：目的探讨自拟活血药方预防不稳定型股骨颈骨折内固定术后下肢深静脉血栓(DVT)的效果。方法选取2018年2月至2021年5月在上饶市中医院接受内固定术治疗的86例不稳定型股骨颈骨折患者作为研究对象,采用简单随机分组法将其分为观察组和对照组,每组各43例。对照组患者术后采用低分子量肝素钙预防,观察组患者在对照组的基础上增加自拟活血药方预防,均持续干预2周。比较两组患者的DVT形成情况、血浆活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、凝血酶时间(TT)、纤维蛋白原(FIB)、D-二聚体(D-D)及不良反应发生情况。结果观察组患者术后DVT发生率低于对照组,差异有统计学意义(P<0.05)。观察组患者干预后的APTT、PT、TT高于对照组,FIB、D-D低于对照组,差异有统计学意义(P<0.05)。两组患者的不良反应发生率比较,差异无统计学意义(P>0.05)。结论自拟活血药方可以改善不稳定型股骨颈骨折患者内固定术后血浆高凝状态,预防下肢DVT形成。Objective To explore the effect of self-made Huoxue Decoction on prevention of deep vein thrombosis(DVT)after internal fixation of unstable femoral neck fractures.Methods A total of 86 patients with unstable femoral neck fractures undergoing internal fixation in Shangrao Hospital of Traditional Chinese Medicine were enrolled between February 2018 and May 2021.According to simple random grouping method,they were divided into observation group and control group,with 43 cases in each group.After surgery,control group was treated with low-molecular weight Heparin Calcium,while observation group was treated with self-made Huoxue Decoction on basis of control group.All were continuously intervened for 2 weeks.DVT formation,plasma activated partial thrombin time(APTT),prothrombin time(PT),thrombin time(TT),fibrinogen(FIB),D-dimer(D-D)and the incidence of adverse reactions in the two groups were compared.Results The incidence of postoperative DVT in observation group was lower than that in control group,and the difference was statistically significant(P<0.05).After intervention,APTT,PT and TT in observation group were higher than those in control group,while FIB and D-D were lower than those in control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Self-made Huoxue Decoction can improve plasma hypercoagulable state and prevent DVT in patients with unstable femoral neck fractures after internal fixation.
