经皮肾囊肿穿刺去顶减压术与后腹腔镜肾囊肿去顶减压术在单纯性肾囊肿中应用效果的比较

摘　　要：目的探讨经皮肾囊肿穿刺去顶减压术与后腹腔镜肾囊肿去顶减压术治疗单纯性肾囊肿的效果。方法回顾性分析2017年4月至2021年5月广东省英德市人民医院泌尿外科收治的49例单纯性肾囊肿患者的临床资料,依据手术方式不同分为经皮组(24例)及腹腔镜组(25例),经皮组患者采用经皮肾囊肿穿刺去顶减压术,腹腔镜组患者采用后腹腔镜肾囊肿去顶减压术,术后随访6个月。比较两组围手术期指标、术后并发症总发生率及复发率。结果经皮组手术时间、术后住院时间均短于腹腔镜组,术中出血量少于腹腔镜组,术后引流量多于腹腔镜组,视觉模拟评分表(VAS)评分低于腹腔镜组,差异有统计学意义(P<0.05);经皮组术后并发症总发生率为4.17%、复发率为0,与腹腔镜组的4.00%及4.00%比较,差异无统计学意义(P>0.05)。结论经皮肾囊肿穿刺去顶减压术与后腹腔镜肾囊肿去顶减压术均可治疗单纯性肾囊肿,但前者手术操作较为简单,术后疼痛感较轻,并发症少,安全性高,值得推广。Objective To investigate the efficacy of percutaneous renal cyst unroofing decompression and retroperitoneal laparoscopic renal cyst unroofing decompression in the treatment of simple renal cyst.Methods The clinical data of 49 patients with simple renal cyst treated in the Department of Urology of Yingde People′s Hospital of Guangdong Province from April 2017 to May 2021 were analyzed retrospectively.According to different operation methods,they were divided into percutaneous group(24 cases)and laparoscopic group(25 cases).The patients in the percutaneous group underwent percutaneous renal cyst puncture and decompression,The patients in the laparoscopic group underwent retroperitoneal laparoscopic renal cyst decapitation and decompression,and were followed up for 6 months.The perioperative indexes,total incidence of postoperative complications and recurrence rate were compared between the two groups.Results The operation time and postoperative hospital stay in the percutaneous group were shorter than those in the laparoscopic group,the amount of intraoperative bleeding was less than that in the laparoscopic group,the postoperative drainage volume was more than that in the laparoscopic group,and the score of visual analogue scale(VAS)was lower than that in the laparoscopic group,with statistical significances(P<0.05).The total incidence of postoperative complications in the percutaneous group was 4.17%and the recurrence rate was 0,there were no significant difference compared with 4.00%and 4.00%in the laparoscopic group(P>0.05).Conclusion Both percutaneous renal cyst decompression and retroperitoneal laparoscopic renal cyst decompression can treat simple renal cyst,but the former has simple operation,less postoperative pain,less complications and high safety,which is worthy of popularization.
