自拟补肾健脾安胎方治疗先兆流产合并亚临床甲减的临床效果

摘　　要：目的探讨自拟补肾健脾安胎方治疗先兆流产合并亚临床甲减的临床效果。方法选取2020年8月至2021年3月江西省妇幼保健院的100例先兆流产合并亚临床甲减患者作为研究对象,采用随机数字表法将其分为试验组(50例)和对照组(50例)。两组均给予黄体酮注射液治疗,对照组患者采用左甲状腺素钠片治疗,试验组在对照组的基础上联合自拟补肾健脾安胎方治疗。两组均治疗30 d。比较两组治疗后的临床疗效及治疗前后的妊娠激素与甲状腺功能指标水平。结果试验组治疗后的临床总有效率为88.00%,高于对照组的70.00%,差异有统计学意义(P<0.05)。两组治疗后的血清血人绒毛膜促性腺激素(HCG)、孕酮(P)、雌二醇(E_(2))水平均高于治疗前,且试验组治疗后的HCG、P、E_(2)水平高于对照组,差异均有统计学意义(P<0.05);两组治疗后的血清促甲状腺激素(TSH)水平均低于治疗前,且试验组治疗后的TSH水平低于对照组,差异均有统计学意义(P<0.05);两组治疗前后的血清游离甲状腺素(FT4)、游离三碘甲状腺原氨酸(FT3)水平比较,差异无统计学意义(P>0.05)。结论自拟补肾健脾安胎方可调节先兆流产合并亚临床甲减患者的甲状功能指标与妊娠激素水平,提升治疗效果。Objective To explore the clinical effect of self-made Bushen Jianpi Antai Decoction in the treatment of threatened abortion with subclinical hypothyroidism.Methods A total of 100 patients with threatened abortion combined with subclinical hypothyroidism in Maternal and Child Health Hospital of Jiangxi Province from August 2020 to March 2021 were selected as the research objects,and they were divided into the test group(50 cases)and the control group(50 cases)according to the random number table method.Both groups were treated with Progesterone Injection,the control group was treated with Levothyroxine Sodium Tablets,and the test group was treated with self-made Bushen Jianpi Antai Decoction on the basis of the control group.Both groups were treated for 30 days.The clinical efficacy after treatment and the levels of pregnancy hormone and thyroid function indexes before and after treatment were compared between the two groups.Results The total clinical effective rate after treatment in the test group was 88.00%,which was higher than 70.00%in the control group,the difference was statistically significant(P<0.05).After treatment,the serum blood human chorionic gonadotropin(HCG),progesterone(P),estradiol(E_(2))levels in the two groups were higher than those before treatment,and the levels of HCG,P,E_(2)in the test group after treatment were higher than those in the control group,the differences were statistically significant(P<0.05).The serum thyroid stimulating hormone(TSH)levels in the two groups after treatment were lower than those before treatment,and the level of TSH in the test group was lower than that in the control group,the differences were statistically significant(P<0.05).There were no significant differences in the levels of serum free thyroxine(FT4)and free triiodothyronine(FT3)between the two groups before and after treatment(P>0.05).Conclusion The self-made Bushen Jianpi Antai Decoction can adjust the thyroid function indexes and pregnancy hormone levels of patients with threatened abortion and subcli
