培元止遗汤联合耳穴贴压治疗儿童夜遗尿脾肾两虚证疗效观察

摘　　要：目的观察培元止遗汤联合耳穴贴压治疗儿童夜遗尿脾肾两虚证的临床疗效。方法将100例儿童夜遗尿脾肾两虚证患儿按照随机数字表法分为2组,对照组50例予常规治疗,治疗组50例在对照组治疗基础上予培元止遗汤联合耳穴贴压治疗。2组均治疗1个月。比较2组治疗前后遗尿频率、膀胱容量、膀胱容量壁厚指数(BVWI)、儿童夜遗尿脾肾两虚证相关中医证候评分,并统计2组疗效及6个月后复发率。结果治疗组总有效率92.00%(46/50),对照组总有效率76.00%(38/50),治疗组疗效优于对照组(P<0.05)。2组治疗后遗尿频率、BVWI均较本组治疗前降低(P<0.05),膀胱容量均增加(P<0.05),且治疗组治疗后遗尿频率、BVWI均低于对照组(P<0.05),膀胱容量高于对照组(P<0.05)。2组治疗后中医证候各项评分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。治疗组复发率6.52%(3/46),对照组复发率26.31%(10/38),治疗组复发率低于对照组(P<0.05)。结论培元止遗汤联合耳穴贴压治疗儿童夜遗尿脾肾两虚证能够显著降低遗尿频率,提升膀胱容量,改善临床疗效,降低复发率。Objective To observe the clinical efficacy of Peiyuan Zhiyi Decoction combined with auricular point sticking(APS)on children with nocturnal enuresis(NE)of spleen-kidney deficiency.Methods Totally 100 children with NE(spleen-kidney deficiency)were randomly divided into the control group(n=50)and the treatment group(n=50),apart from the conventional therapy,and the treatment group received Peiyuan Zhiyi Decoction combined with APS.Treated for 1 month,the aim was to compare NE frequency,bladder volume(BV),bladder volume wall thickness index(BVWI),traditional Chinese medicine(TCM)syndrome score;the curative effect and the recurrence rate on month 6 were assessed.Results The overall effective rate was 92.00%(46/50)in the treatment group,higher than that of 76.00%(38/50)in the control group(P<0.05).After treatment,NE frequency and BVWI in groups were decreased(P<0.05),the treatment group was lower than the control group(P<0.05);BV in groups was increased(P<0.05),which was higher in the treatment group than in the control group(P<0.05);TCM symptom scores in groups were decreased(P<0.05),the treatment group was more prevalent(P<0.05).The recurrence rate in the treatment group was lower than that in the control group(6.52%[3/46]vs 26.31%[10/38],[P<0.05],respectively).Conclusion For children with NE(spleen-kidney deficiency),Peiyuan Zhiyi Decoction combined with APS is significantly capable of reducing NE frequency,increasing BV,with better clinical efficacy and lower recurrence rate.
